摘要
目的 观察小切口手法碎核白内障囊外摘除人工晶体植入手术的疗效.方法 对650例725眼行小切口手法碎核白内障囊外摘除人工晶体植人术.术中小切口下充分水分离白内障核后用晶状体注水圈匙托起晶状体核,再用虹膜回复器把晶状体核向下压碎,并用虹膜同复器及注水圈匙一起把碎核夹出的方法摘出晶状体核,植入人工晶体.观察碎核情况、术后视力和手术并发症.结果 术后2天裸眼视力≥0.5者630眼占86.90%.术后角膜内皮轻度水肿53眼占7.31%,角膜平均散光与术前对比相差0.50～0.75D.结论 作角巩膜小切口白内障挤压碎核夹出联合人工晶体植人手术不需要特殊器械,操作简单,术后视力恢复快,角膜散光少、角膜水肿发生率低、并发症少,值得临床推广.
Objective To evaluate the efficacy of small incision and manual nuclear chopping combined with extracapsular cataract extraction and intraocular lens implantation. Methoda 650 cataract patients （725 eyes） who had undergone small incision and manual nuclear chopping combined with extracapsu- lar cataract extraction and intraocular lens implantation were observed. During the surgery, cataract nucleus was separated by water via small incisions and lens nucleus was lifted by infusion lens loop, then the lens nucleus was chopped by an irisrepositor; lens nucleus was extracted and intraocular lens was implanted . The chopping nucleus situation, postoperative visual acuity and complications were observed. Results The corrected visual acuity=0.5 was in 630 eyes （86.90%） two days after surgery, slight corneal edema in 53 eyes （7.31 %）. The difference of average astigmatism was 0.50D to 0.75D. Conclusions Small incision and manual nuclear chopping combined with intraocular lens implantation doesn＇ t need special instrument and is simple. This procedure can reduce postoperative astigmatism, comeal edema, and other complications! shorten the period of visual rehabilitation; and have the same efficacy as that of phacoemulsiflcation.
