摘要
目的:分析盐酸戊乙奎醚治疗脓毒性休克临床疗效。方法:40例脓毒性休克住院患者随机分为对照组(n=20)和盐酸戊乙奎醚治疗组(PH组,n=20)),对照组予山莨菪碱(654-2)治疗;PH组予盐酸戊乙奎醚治疗。观察患者治疗效果及末梢转暖时间,给药后2h、6h、12h时点患者心率、平均动脉压、瞳孔、动脉血氧饱和度、肠鸣音及血乳酸含量变化。结果:PH组、对照组总有效率比较,差异无统计学意义(P>0.05)。PH组末梢转暖持续时间明显延长,两组差异有统计学意义(P>0.05)。与治疗前比较,治疗后两组患者血乳酸含量下降、动脉氧饱和度明显改善;PH组心率、平均动脉压、瞳孔、肠鸣音变化差异无统计学意义。两组间比较,PH组血乳酸水平下降更明显,差异有统计学意义(P<0.05),且两组在2h、6h时点心率、瞳孔、肠鸣音变化比较,差异有统计学意义(P<0.05)。治疗后,两组的APCACHEⅡ评分、SOFA评分较治疗前减少(P<0.05),且PH组评分减少比对照组更明显(P<0.05)。结论:盐酸戊乙奎醚可明显改善休克患者微循环,无明显增加心率、抑制肠蠕动副作用,是脓毒性休克患者较理想的血管活性药。
Objective:To analyze therapeutic effectiveness of penehyclidine hydrochloride used to treat patients with septic shock.Methods:A total of 40 patients with septic shock were randomly divided into control group(n=20) and penehyclidine hydrochloride treated group(PH group,n=20).Control group were given 654-2 while PH group were given penehvclidine hydroohhloride.Therapeutic effectiveness were observed as well as the peripheral warming time,the heart rate(HR)of 2,6 and 12 hours after treatment,mean arterial blood pressure(MABP),diameter of the pupil,saturation of pulse oxygen(SaO2),intestinal gurgling sound(IGS) and the level of lactic acid in blood(BLA).Results:There was no statistically significant difference in the total efficacy between the two groups(P 0.05).Continuous time of peripheral warming in PH group was significantly longer than that in control group.SaO2 and the level of BLA in two groups were improved significantly after treatment(P0.05);whereas the changes of HR,MABP,diameter of the pupil and IGS in PH group were of no statistical significance after treatment.Compared with control group,the level of BLA was much lower in the PH group(P0.05).There were significantly difference in the heart rate,diameter of the pupil and intestinal gurgling sound at 2h and 6h between the two groups(P0.05).After treatment,APCACHE and SOFA score in all patients were lower than before treatmen(tP0.05),and APCACHE and SOFA score in PH group were obviously lower than those in control(P0.05).Conclusion:Penehyclidine hydrochloride could improve the microcirculation of patients with septic shock,and side effects are less such as increasing the heart rate and inhibiting the intestinal motion.So,penehyclidine hydrochloride could be one of the ideal vasoactive drugs in treatment for septic shock.
