翼点入路基底静脉前段的显微解剖与临床价值

Microsurgical anatomy of the anterior segment of basal vein and the clinical significance via pterional approach

摘要 目的对基底静脉前段的显微解剖变异进行研究,为临床神经外科手术提供形态学依据,以便提供更好的术中保护。方法利用20具(40侧)尸头标本,动、静脉分别用红蓝色乳胶灌注,先模拟翼点入路进行观察,然后去除颅盖骨,在游离的脑标本上观察、测量基底静脉前段及其属支并进行相关数据统计。结果 4侧基底静脉前段缺如,36侧紧贴视束后行,25侧直接绕环池后行,7侧直接汇入蝶顶窦。大脑脚静脉和侧脑室下静脉汇合成基底静脉中段,绕环池后行,前段和中段不连续;4侧基底静脉前段汇入蝶顶窦,前段和中段连续。结论熟悉基底静脉前段的显微解剖变异,在翼点入路处理鞍区、上斜坡肿瘤和基底动脉分叉动脉瘤的手术过程加以更好的保护,能减少出血和术后并发症的发生。 Objective To offer morphology evidence for clinical neurosurgery in order to protect anterior segment of basal vein in operation by studying the variation of its microsurgical anatomy. Methods Twenty adult cadaveric skulls (forty hemispheres) were perfused with red and blue latex. Basal veins and the tributaries were observed and measured after simulating pterional approach and opening the cranium. Results Anterior segments of basal veins were absent in 4 sides, ran posteriorly along fasciculus opticus in 36 sides, ran posteriorly around ambient cistern in 25 sides and flew into sphenoparietal sinus in 7 sides. Peduncular veins and inferior ventricular veins converging on the middle segments of basal veins and ran posteriorly, the anterior and middle segments of basal veins were discontinuous. In 4 sides anterior segments of basal veins flowed into sphenoparietal sinus and the anterior and iniddle se^nents were continuous. Conclusion Familiarity with the variation of microsurgical anatomy of the anterior segments of basal veins can provide better protection and decrease hemorrhage and the possible postoperative complications when dealing with tumor in saddle area and the upper clivus and basilar apex aneurysm.
