摘要
目的探讨Ⅰb2-Ⅱb期宫颈癌患者术前经髂内动脉介入化疗栓塞的临床应用价值。方法 54例宫颈癌Ⅰb2-Ⅱb期患者,术前行超选择性插管化疗栓塞,10～14d后行宫颈癌根治术,分析其手术切除率、术中出血量及疗效,并与43例单纯手术病人进行对照。结果 2组患者的手术时间、术中出血量、术后留置尿管时间、术中术后并发症、宫旁切缘残留、阴道切缘残留差异均无统计学意义(P>0.05)。化疗栓塞组临床总有效率为88.9%,其3年和5年生存率分别为90.7%和72.2%,明显高于对照组(79.1%和46.5%)。栓塞组常见的不良反应有恶心及食欲下降等胃肠道反应。无严重并发症发生。结论Ⅰb2-Ⅱb宫颈癌术前行动脉介入化疗,能缩小宫颈癌病灶,提高手术切除率,减少临床转移复发,明显提高宫颈癌患者的生存率。
Objective To investigate the clinical value of preoperative intra-internal iliac arteria chemoembolization in stageⅠb2Ⅱb cervical cancer.Methods From Feb 2002 to Jul 2004 54 patients with stageⅠb2-Ⅱb cervical cancer performed preoperative super selective catheterization chemoembolization（embolism group） for 10-14 days,then radical resection of cervical cancer were underwent.The operative rate,intraoperative bleeding and therapeutic effects were analyzed.43 cases of stageⅠb2-Ⅱb cervical cancer underwent simple operation （control group）.All the patients were followed-up for 5 years.Results There were no significant difference between embolism group and control group in terms of surgical time,blood loss,length of urinary canal,complication of operation,cutting edge remaining of cervix and vagina.The total effective rate of embolism group was 88.9%.The 3-year and 5-year survival rates of the embolism group were 90.7% and 72.2% respectively,higher than that of the control group,which were 79.1% and 46.5% respectively.The frequent side effect in embolism group was nausea and vomiting,no critical complication occurred.Conclusion Preoperative uterine artery chemoembolization is an effective adjunctive therapy in stageⅠb2-Ⅱb cervical cancer.
