摘要
目的评价64层螺旋CT血管成像对脑动脉瘤的诊断价值。方法对自发性蛛网膜下腔出血患者共27例进行CTA检查,应用容积重建(VR)、最大密度投影(MIP)、数字减影重建(NEURO-DSA)对脑血管显示情况进行评价,24例CTA异常病人均进行DSA对照检查,其中20例行DSA介入治疗,3例进行手术治疗。结果27例中CTA诊断为脑动脉瘤24例,3例未见动脉瘤改变,对24例进行DSA对照结果:后交通动脉瘤14例,前交通动脉瘤3例,大脑中动脉动脉瘤2例,大脑后动脉动脉瘤1例,后交通动脉瘤同时存在2例,后交通与大脑后动脉瘤同时存在1例,1例DSA结果未见异常改变,诊断准确率为97%,动脉瘤数量为26个。结论64层螺旋CT血管成像快速、准确,对自发性蛛网膜下腔出血诊断与DSA基本一致,能够为临床提供准确诊断结果,可以做为脑动脉瘤诊断首选依据。
Objective To evaluate 64-slice spiral CT angiography for the diagnosis of cerebral aneurysms. Methods CTA was used for 27 cases of spontaneous subarachnoid hemorrhage. The cerebral blood vessels were evaluated by using volume rendering(VR), maximum intensity projection(MIP)and digital subtraction reconstruction(NEURO-DSA),and the DSA control examination was conducted in 24 cases of abnormal CTA,including the DSA interventional therapy for 20 cases and surgery for 3 cases. Results In 27 cases,24 were diagnosed as cerebral aneurysm and 3 cases showed no change in aneurysm.DSA control examination of 24 cases showed 14 cases of posterior communicating artery aneurysm, 3 of anterior communicating artery aneurysm,2 of middle cerebral artery aneurysm, one of posterior cerebral artery aneurysm,2 of co-existing anterior posterior communicating artery aneurysm,one of co-existing posterior communicating and posterior cerebral artery aneurysm and one showed no abnormal changes by DSA,witb the diagnostic accuracy rate of 97% and the aneurysm number of 26. Conclusion The 64 slice CTA can make rapid and accurate angiography and the CTA diagnosis of spontaneous subarachnoid hemorrhage is basically consistent with the DSA diagnosis,thus providing accurate clinical diagnosis,and it can be used for the first choice for diagnosis of cerebral aneurysms.
