摘要
目的：探讨应用人工股骨头置换术治疗老龄不稳定型股骨粗隆间骨折的疗效。方法：自2000年6月-2009年1月，采用人工股骨头置换术治疗高龄不稳定型股骨粗隆间骨折患者42例，分别应用骨水泥人工双极股骨头（27例）和非骨水泥生物型长柄人工双极股骨头（15例），分析术前与术后Harris评分及术周、术后并发症等。结果：所有患者手术顺利进行，术前Harris评分39．7±11．5，术后Harris评分90．5±6．5，术后与术前Harris评分比较t=11．4461，P〈0．05。术后并发症，脱位1例，精神症状2例。本组病例无肺栓塞、深静脉血栓形成、感染等并发症发生。术后随访6～25个月，平均12．2个月，X线检查示假体位置良好。结论：人工股骨头置换术是治疗高龄不稳定型股骨粗隆间骨折的一种有效方法。
Objective To explore retrospectively clinical effect of treatment of unstable intertrochanteric fracture with hip hemiarthroplasty in the elder. Methods Forty two cases with unstable intertrochanteric fracture had been treated with hip hemi - arthroplasty （HHA） in the advanced ages between June 2000 and January 2009. 27 out of 42 cases had been performed the cemented bipolar femoral head prosthesis, and 15 out of 42 cases had been performed the 1 ong - stem non - cemented bipolar femoral head prosthesis. These had been analyzed the operating duration, the total amount of blood loss, the amount of blood transfusion necessary, pre - operation functional scores of hips and post - operation functional scores, the peri - operative and post - operative complications and so on. Restilts The operations had been performed successfully. The pre - operation functional scores of hips 39. 7 ± 11.5 and post - operation functional scores 90. 5 ± 6. 5. The difference had statistical significance between post - operation functional scores of hips and pre - operation functional scores （t = 11. 4461, P 〈 0. 05 ） . The peri - operative and post - operative complications were happened to 3 cases, the dislocation of hips in 1 cases, and mental symptoms in 2 cases respectively, and no others complications in the series. The mean followed up period was 12.2 months, ranging from 6 to 25 months, and the position of the components were radiographic satisfactory. Conclusions HHA is an effective way to treat unstable intertrochanteric fracture in the elder.
