摘要
目的探讨高分辨率CT对早期周围性小肺癌(直径≤3cm)的诊断价值。方法收集30例肺癌病例,经手术、穿刺活检的病理结果和临床观察证实。每例病人均行常规螺旋CT扫描,再对病灶行高分辨扫描,其中20例行增强后高分辨重复扫描。对病灶大小、部位、形态、边缘、密度、强化前后CT值及强化范围、以及病灶与支气管的关系等信息进行观察与分析。结果30例早期肺癌的主要CT表现:全部为孤立型小结节,30例有分叶征;30例有小毛刺征;15例有胸膜凹陷征;16例有血管集束征;9例有支气管气相和空泡征。结论高分辩率CT+动态增强扫描能更好显示病灶内部结构和边缘形态,可提高早期周围型肺癌的诊断率,具有重要的临床应用价值。
Objective To analyze HRCT findings of small peripheral lung cancer. Methods Thirty cases of small peripheral lung cancers were proved by clinical observation and pathology. All cases were examined by conventional helical scan first, and then by high-resolution scan. 20 cases were given contrasted SCT scans, and then were repeated HRCT. The imaging findings of 30 cases with lesion size, location, shape, boundary, internal density and enhanced patterns were observed. Results The early manifestations of small peripheral lung cancer were included separated-lobules （30）, little speculation （30）, pleural-depress （15）, blood vessel concentration （16） and air-bronchium and empty-bubble （9）. Conclusion It is valuable of HRCT and contrast-enhancement on small peripheral lung cancer better.
