摘要
目的探讨彩色多普勒超声检查在下肢深静脉血栓形成(DVT)中的临床诊断价值。方法应用彩色多普勒超声检测临床疑为DVT患者58例,后均经X线血管造影证实,观察血栓形成的部位、回声特征及栓塞程度,并追踪观察溶栓治疗后血栓再通情况。结果58例下肢深静脉血栓,彩色多普勒超声诊断56例,诊断符合率96.5%,漏诊1例胫前静脉血栓和1例胫后静脉血栓,对髂外静脉血栓、股静脉血栓、腘静脉血栓无1例漏诊。根据血栓分期,急性期20例,其中完全闭塞12例,不完全闭塞8例;亚急性及慢性期36例,其中完全闭塞10例,不完全闭塞26例。结论彩色多普勒超声对下肢深静脉血栓的部位、分期、管腔阻塞情况可做出较为肯定的诊断,同时可通过动态观察其演变情况,判断临床治疗效果。
Objective To discuss the diagnostic value of colored Doppler flow imaging（CDFI） used in the deep venous thrombosis in lower extremities. Methods 58 patients suspected of DVT were observed by CDFI for the thrombosis site, the echo characteristic and the degree of embolization,follow up the recanalization of thrombosis after thrombolytic therapy.Results Color Doppler ultrasound examined 56 cases of DVT,the corresponding rate was 96.5%（56/58）,one case of anterior tibial vein thrombosis and one case of posterior tibial vein thrombosis were missed diagnosis.None case of deep venous thrombosis which in external iliac vein, femoral vein, popliteal vein was missed diagnosis.There were 20 cases of acute thrombosis, in which 12 cases were total occlusion and 8 cases were incomplete occlusion.There were 36 cases of sub-acute and chronic thrombosis, in which 10 cases were total occlusion and 26 cases were incomplete occlusion.Conclusions CDFI can make some degree of certain diagnosis in site, stage, luminal obstruction of DVT, and observe the evolution of thrombosis to judge the effect of clinical treatment.
