Clinical application of extracorporeal lipid-apheresis technology

摘要 血脂异常[胆固醇、甘油三酯、低密度脂蛋白、脂蛋白(a)升高或高密度脂蛋白降低]是导致动脉粥样硬化性疾病如冠心病、脑血管疾病的主要危险因素,因此积极治疗血脂异常对于预防心脑血管疾病至关重要。除了饮食控制和药物治疗外,体外血脂分离技术被认为是最有效和最有前途的治疗方法。现今已有血浆置换法、双膜滤过法、免疫吸附法、化学吸附法、肝素沉淀法和体外血浆脂蛋白过滤器吸附过滤疗法六大类型的体外血脂分离技术应用于临床,并且各有其优缺点。本文就这些技术的治疗原理及临床应用现状做一简述。 Studies have identified dyslipidemia(elevation of serum total cholesterol,triglycerides,low-density lipoprotein cholesterol and lipoprotein(a),and reduction of high-density lipoprotein cholesterol) as putative risk factors for atherosclerotic diseases such as coronary artery disease(CAD) and cerebrovascular disease(CVD).Therefore,it is essential to treat dyslipidemia actively in preventing the progression of CAD and CVD.In addition to diet and drug therapy,extracorporeal lipid-apheresis has been considered the most effective and most promising method of treatment.Five different techniques of extracorporeal lipid-apheresis are in current use: plasma exchange,double filtration plasmapheresis,immunoadsorption,chemical adsorption,heparin extracorporeal LDL precipitation system,delipid extracossoseal lipoprotein filter from plasma.And each has its own advantages and disadvantages.In this paper,there is a brief depiction about the treatment principal and the status quo of clinical application of these techniques.
