摘要
目的了解上海地区志贺菌的耐药特征,明确其所携带ESBLs的基因型。方法用K-B纸片扩散法对收集到的216株临床分离的志贺菌进行耐药性检测和ESBLs确证试验;采用接合试验了解耐药性是否由质粒介导;用CTX-M通用引物对产ESBLs的菌株的耐药基因进行PCR扩增。结果药敏结果显示志贺菌对萘啶酸、氨苄西林、氨苄西林-舒巴坦和复方磺胺甲口恶唑的敏感率均低于50%,对头孢噻肟的敏感率为81.5%,ESBLs的检出率为18.5%(40/216),其中87.5%(35/40)的ES-BLs接合试验阳性。PCR扩增显示CTX-M基因阳性率为90%(36/40),序列分析证实其型别主要为CTX-M-14(20/40)、CTX-M-15(13/40)和CTX-M-3(3/40)。结论上海地区志贺菌呈多重耐药,对第三代头孢菌素耐药性有增高趋势。上海地区志贺菌所携带ESBLs的基因型主要为CTX-M-14、CTX-M-15和CTX-M-3。
Objective To analyze the resistant pattern and the extended-spectrum β-1actamases genotypes of clinical isolates of Shigella in Shanghai. Methods Susceptibility testing was performed by K B disk diffusion method. Transferability of resistance to cefotaxime was examined by conjugation test. PCR method was employed to detect the genotypes of ESBLs-producing strains using CTX-M and SHV universal primers. The PCR products were analyzed by DNA sequencing. Results Less than 50% of these isolates were susceptible to nalidixic acid, ampicillin, ampicillin-sulbactam and trimethoprim-sulfamethoxazole. And 81.5% of these isolates were susceptible to eefotaxime. ESBLs were identified in 18. 5% (40/216) of Shigella isolates and 87.5% (35/40) of them were conjugated to Escherichia coli J53 Az. Most (90%, 36/40) of ESBLs-producing strains were detected by CTX-M primers. The genotypes of these strains included CTX-M-14 (20/40), CTX-M-15 (13/40) and CTX-M-3 (3/40). Conclusions The Shigella strains isolated in Shanghai were multi drug resistant. The main genotypes of ESBLs were CTX-M-14, CTX-M-15 and CTX-M-3.
