[^(11)C]-β-CFT脑多巴胺转运体PET显像在帕金森病诊断中的应用 被引量:11

Application of [^(11)C]-β-CFT Dopamine Transporter PET Imaging in Diagnosing of Parkinson's Disease

摘要 目的:探讨[11C]-β-CFT多巴胺转运体(DAT)PET显像在帕金森病(PD)早期诊断及病情严重度评估的应用价值。方法:对早期PD组、晚期PD组和正常对照组进行[11C]-β-CFT基底节区DATPET显像,比较3组间基底节DAT摄取指数差异,分析PD组基底节区DAT摄取指数与UPDRS评分的相关性。结果:PD患者基底节区DAT摄取指数显著降低;其中单侧PD患者症状/同侧基底节区DAT摄取指数明显低于对照组。PD组基底节各亚区DAT摄取指数与UPDRS运动评分呈显著负相关。结论:[11C]-β-CFT基底节DATPET显像有助于PD的早期诊断及病情严重度的评估。 Aim: To evaluate the application of 2β-carbomethoxy-3β-(4-fluorophenyl)-(N-^11C-methyl) tropane ([^11C]-β-CFT) dopamine transporter (DAT) PET imaging in diagnosing of Parkinson' s disease(PD) at early stage and assess the severity of PD. Methods: 10 PD patients with early stage, 6 PD patients at advanced stage and 5 age-matched healthy controls were enrolled in this study. DAT PET imaging in brain were performed and analyzed for all above subjects one hour after injection of [^11C]-β-CFT. The ratio of (ROI- cerebellum)/cerebellum was measured and compared among different groups, and evaluated by correlation analysis with United Parkinson Disease Rating Scale Motor Score (UPDRS Ⅲ). Comparison of [^11C]-β-CFT uptake between early or advanced PD and the control group was also applied by using statistical parametric mapping (SPM) analysis. Results: In the PD group at early stage, the DAT [^11C]-β-CFT uptake in the caudate, anterior and posterior putamen was reduced to 51.9%, 37.9% and 25.7% of that in the control group respectively. There was a significant reduction of [^11C]-β-CFT uptake in the ipsilateral (preclinical) striatum of 8 hemiparkinsonian patients at early stage. In the PD patients at advanced stage, the above corresponding value were reduced to 34.8%, 21.0% and 13.2% respectively. The [^11C]-β-CFT uptake in striatum of all PD patients showed significantly negative correlation with UPDRS motor scores. By means of SPM analysis, there was a significant reduction of DAT binding in the contralateral striatum of PD patients at early stage and in the bilateral striatum of patients at advanced stage, especially in posterior putamen contralateral to the affected fimbs. Conclusion: [^11C]-β-CFT could be served as a sensitive biomarker for detecting nigrostriatal dysfunction in PD at early stage and evaluating the severity of PD.
