参附注射液对失血性休克大鼠微循环血流量的影响 被引量:3

Protective effects of Shenfu injection on gastric mucosa and kidney in hemorrhagic shock: experiment with rats

摘要 目的探讨参附注射液对失血性休克大鼠胃肠道、肾脏等损伤的影响。方法30只健康雄性SD大鼠随机均分为3组:①对照组,麻醉后动静脉插管,插管后4h 15min用激光多普勒血流仪测定胃黏膜血流量(GMBF),肾黏膜血流量(KMBF),夹取一块胃组织作镜检,采集肾脏标本,采用透射电镜观察肾的微观结构;②失血性休克组:放血降低动脉压,建立休克模型,输注失血量3倍的生理盐水复苏;复苏开始后3h检测GMBF和KMBF,并采集肾脏标本;③参附注射液治疗组:模型复制成功后输注参附注射液10ml/kg,再输入生理盐水补充至3倍失血量,于复苏开始后3h检测GMBF和KMBF.并采集肾脏标本。结果失血性休克组GMBF显著低于对照组(P〈0.01),并可见胃黏膜细胞坏死、脱落、局部溃疡形成。参附注射液治疗组GMBF显著高于失血性休克组(P〈0.01),胃黏膜损伤显著较轻,与对照组比较无明显差异。失血性休克组KMBF显著低于对照组(P〈0.01),肾脏颜色红。参附注射液治疗组GMBF显著高于失血性休克组高(P〈0.01),胃黏膜损伤显著较轻,与对照组比较无明显差异。结论参附注射液能够通过提高失血性休克大鼠胃黏膜、肾脏血流量,改善胃黏膜、肾脏缺血再灌注损伤。 Objective To investigate the protective effects of Shenfu injection on gastric mucosa and kidney in hemorrhagic shock. Methods Thirty healthy male Sprague Dawley rats were randomly assigned into 3 equal groups. (1) Control group. Anesthesia and intubation into the left common carotid artery and right external jugular vein were conducted. 4h 15min after the intubation the gastric mucosal blood flow (GMBF) and kidney mucosal blood flow (KMBF) were examined with laser Doppler perfusion imager, a piece of gastric tissue was nipped, and then the kidneys were taken out to undergo transmission electron flowmetry (LDF). (2) Hemorrhagic shock group. Bloodletting was conducted after the intubation and normal saline (NS) of the volume 3 times as the blood loss was infused. Three hours after the resuscitation GMBF and KMBF were examined by LDF and the kidneys were taken out to undergo transmission electron microscopy. (3) Shenfu injection group. Shenfu injection 10 ml/kg was injected and then NS was infused till the volume reached the level 3 tines as the blood loss. Three hours later GMBF and KMBF were examined and the kidneys were taken out to undergo transmission electron microscopy. Results The GMBF level of the hemorrhagic shock group was significantly lower than that of the control group (P〈 0.01). Necrosis, ulceration, and exfoliation of epithelial cells were seen in the gastric mucosa. The GMBF level of the Shenfu injection group was significantly higher than that of the hemorrhagic shock group (P 〈 0.01), but not significantly different from that of the control group (P 〉0. 05). The gastric mucosa injuries of the Shenfu injection group were greatly milder compared with the hemorrhagic shock group. The KMBF level of the hemorrhagic shock group was significantly lower than that of the control group (P 〈 0.01) , and the color of kidney was red. The KMBF level of the Shenfu injection group was significantly higher than that of the hemorrhagic shock group (P〈 0.01), however, not significantly different from that of the control group (P 〉 0. 05). The color of kidney of the Shenfu injection group was red, and the kidney injuries were greatly milder. Conclusion The Shenfu injection, through enhancing the GMBF and KMBF, can ameliorate the ischemic-reperfusion injuries in the gastric mucosa and kidney in hemorrhagic shock.
