摘要
探讨内源性糖皮质激素在酒精性股骨头缺血性坏死骨细胞凋亡中的作用。选用90只家兔,随机分为3组。采用给予灌胃法实验组家兔含乙醇50%的烈性白酒8mL/kg·d,药物干预组等量白酒加氨鲁米特25mg/kg·d,对照组等量生理盐水,1~6个月。化学发光法测定血清皮质醇含量,原位杂交检测GCRmRNA表达,TUNEL法检测骨细胞凋亡。结果表明,家兔血清中皮质醇变化为:实验组(5.49±1.38)μg/dL>干预组(2.15±0.49)μg/dL>对照组(0.73±0.24)μg/dL,3组之间差异均有显著性(P<0.01)。实验组股骨头骨细胞2个月时GCRmRNA弱表达,3个月弱表达或不表达,6个月时不表达;干预组1~3个月时中等表达,6个月时GCRmRNA弱表达;对照组为中等表达。实验组出现大量骨细胞凋亡,以软骨下区最为明显,2个月时实验骨组细胞凋亡数为(107.6±25.87)×10-3,明显高于干预组(60.83±25.34)×10-3和对照组(46.23±23.00)×10-(3P<0.05,P<0.01),但干预组与对照组之间差异无显著性。随着时间推移,凋亡细胞逐渐增多,3、6个月时实验组分别为(195.53±15.72)×10-3和(392.56±26.39)×10-3、干预组分别为(99.20±20.64)×10-3和(144.70±27.63)×10-3、对照组分别为(41.56±22.85)×10-3和(44.09±24.26)×10-3,3组间差异均有显著性(P<0.05,P<0.01)。结果表明,饮酒后可明显增加内源性糖皮质激素的释放,在酒精性ANFH骨细胞凋亡的过程中起重要作用。
The effects of glucocorticoid on osteocyte apoptosis in avascular necrosis of the femoral head induced by alcoholism were studied. In the experiment, 90 rabbits were divided into three groups randomly. The experimental group were administered 8 mL/ (kg. d) liquor (50 % alcoholicity) by gastric-filling, the intervention group were given isodose liquor and 25 mg/ (kg·d) aminoglutethimide, and isodose NS administered in the control group. After 1-6 months administration of all groups, serum cortisol content of the rabbits was measured by chemiluminescence, the expression of GCRmRNA was studied by hybridization in situ, and the change of osteocytes apoptosis was measured by TUNEL. The measured data were as follows: the change of serum cortisol were the experimental group (5.49±1.38 ug/dL )〉 the intervention group (2.15±0.49 ug/dL) 〉 the control group (0.73±0.24 ug/dL), which showed that there was significant difference among the three groups (P〈0.01); the expressions of glucocorticoid receptor mRNA in the osteoeytes of the femoral head were as follows: the experimental group expressed weakly in the second month,weakly or negatively in the third month, and negatively in the sixth month, the intervention group displayed medium expression in the first to the third month and displayed weak expression in the sixth month, and the control group presented medium expression in the first to the sixth month; the apoptosis osteocytes in the experimental group was striking in the subchontral region, in the second month, the number ofapoptosis osteocytes in experimental group ( 107.60±25.87)×10^3 was significantly increased than that in the control group (46.23 ±23.00 )×10^3 or the intervention group (60.83 ±25.34 )×10^3(P〈0.05,P〈0.01), but there was no significant difference between the control group and the intervention group (P〉0.05), as time passed by, the apoptosis osteocytes in the experimental group were (195.53 ± 15.72)×10^3 and (392.56± 26.39) ×10^3 respectively in the third and in the sixth month, the intervention group were (99.20±20.64)×10^3 and (144.70±27.63)×10^3 respectively, and the control group were (41.56±22.85)×10^3 and(44.09±24.26)×10^3 respectively, which showed that there was significant difference among the three groups ( P〈0.01). The experiments proved that the secretion of endogenous glucoeorticoid would increase remarkably after drinking which might play a key role in the process of osteocytes apoptosis in avascular necrosis of the femoral head by alcoholism.
