宁波地区医院肠球菌的临床分布及耐药研究 被引量:1

摘要 目的了解医院肠球菌在临床分布状况及利奈唑胺等抗菌药物对肠球菌的抗菌活性,为临床治疗肠球菌感染合理选择抗菌药物提供指导。方法对本院各种标本分离获得的肠球菌用VITEK微生物自动鉴定仪进行种的鉴定和药物敏感性试验。用WHOVET5软件对统计结果进行分析。结果三年共检出706株肠球菌,其中粪肠球菌432株占肠球菌61.2%,屎肠球菌241株占34.1%,鹑鸡肠球菌33株占4.7%。粪肠球菌对呋喃妥因、青霉素、四环素、莫西沙星、左氧氟沙星的耐药率分别为5.8%、22.2%、71.0%、16.7%和23.8%,而屎肠球菌的耐药率除四环素外均高于粪肠球菌。本组试验发现3株耐万古霉素肠球菌,耐药率为0.43%,肠球菌对利奈唑胺的敏感率为100%。结论肠球菌属细菌在医院内感染逐年增加,耐药率在不断上升,泛耐株在不断增多,本组试验检出3株耐万古霉素肠球菌(VRE)。未发现耐利奈唑胺肠球菌,利奈唑胺是目前治疗泛耐株肠球菌特别是治疗耐万古霉素肠球菌感染的最佳选择,但必须合理使用,同时要密切监测肠球菌的耐药趋势,防止耐药菌株的产生。 Objective To investigate the clinical distribution of enterocoeci and their drug resistance to antihacterials (e. g. linezolid), so as to provide guidance to use antibiotics rationally in clinical therapy. Methods VITEK automatic microbial identification system was applied to performing species identification and drug susceptibility test upon the enterococci isolated from various clinical samples in our hospital from Jan 2005 to Dec 2007. WHONET5 software was adopted to perform statistical analysis on the test results. Results In the three years,706 strains of enterococci were isolated,in which 432 strains (61.2%) were Enterococcus faecalis,241 strains (34.1%) were Enterococcus faecium,and 33 strains (4.7%) were Enterococcus gallinarum. The resistance rates of Enterococeus faecalis to nitrofurantoin, penicillin, tetracycline, moxifloxacin, and levofloxacin were 5. 8 %, 22. 2%, 71.0 %, 16.7% and 23.8% respectively. And the resistance rates of Enterococeus faecium were higher than those of Enterococcus faecalis to most antibiotics except tetracycline. In this study, 3 strains (0.43 %) of vaneomycin-resistant Enterococci were found,while the sensitivity rate of enterocoeci to linezolid accounted for 100%. Conclusion Enterococcus infection in hospitals is increasing year by year; the resistance rate of enterococci keeps on rising, and the pan-resistant strains are gadually increasing. In this study, 3 strains of vaneomycin-resistant Enterococci were found, while no linezolid-resistant enterococci strain was isolated. Linezolid is optimal antibiotic to treat pan-resistant Enterococeus infection especially the infection of vancomycin-resistant enterococci. However,its use must be reasonable so as to avoid the production of resistant strains.
