摘要
目的:采用扩散张量成像(DTI)技术研究脑星形胶质细胞瘤周围区域的细微结构改变,探讨其确定肿瘤边界的价值。方法:将28例经手术病理证实的星形胶质细胞瘤患者分为低度恶性星形细胞瘤和恶性胶质瘤两组。利用DTI检测肿瘤区、肿瘤周围区和健侧脑组织区,计算各向异性(FA)和平均扩散系数(ADC)及脑白质纤维束的情况。结果:28例星形胶质细胞瘤的3个不同区域DTI数值有显著性差异。周围区域DTI各值的均数在低度恶性和恶性胶质瘤之间进行比较,ADC、FA值的差异均有统计学意义。FA图显示低度恶性星形胶质细胞瘤多以推移周围邻近白质为主,与邻近白质边界清楚,而恶性胶质瘤组则以破坏与浸润白质束为主,边界相对模糊。结论:应用DTI成像可能对确定星形胶质细胞瘤的边界具有一定的临床意义。
Objective:To investigate the subtle structural changes in the peri-tumoral region with MR diffusion tensor imaging (DTI) technique, so as to study its value in defining the boundary of brain astrocytoma. Methods:Tweenty-eight patients with brain glioma proved by surgery and'pathology were classified into two groups:low grade malignancy and malignant. All of the patients underwent MR DTI, the fractional anisotropy (FA) and average diffusion coefficient (ADC) values were calculated and normal white matter tract was investigated on FA map imaging in order to study the manifestations of regions in tumor,peri-tumoral and contra lateral normal brain. Results: Of the 28 patients with astrocytoma, the DTI values of 3 different regions showed significant differences. When the peri-tumoral regions of low grade malignant and malignant tumors were compared,the ADC and FA values showed significant statistical differences. On FA map of low grade astrocytoma,the white matter tract mainly showed displacement with well-defined border, whereas invasion and infiltration of the white matter tract with relatively ill-defined border were showed in malignant gliomas. Conelasion:DTI might be helpful to define the border and evaluate the pathology grade of brain astrocytoma.
