摘要
目的研究微粒体甘油三酯转移蛋白(MTP)基因多态性与2型糖尿病合并脂代谢紊乱患者调脂疗效的关系。方法 428例合并高胆固醇血症的2型糖尿病患者,口服辛伐他汀20mg/d,疗程12周,分别在服药前和服药后检测患者血脂等生化指标。应用限制性片段长度多态性聚合酶链反应(PCR-RFLP)和PCR-双脱氧链终止测序法分析MTP-493基因型。结果 (1)MTP-493T/T基因型患者的甘油三酯(TG)和极低密度脂蛋白胆固醇(VLDL-C)较MTP-493G/T基因型和MTP-493G/G基因型患者明显升高:TG分别为(2.89±1.83)mmol/L,(1.58±0.88)mmol/L,(1.68±1.25)mmol/L。VLDL-C分别为(0.69±0.32)mmol/L,(0.52±0.26)mmol/L,(0.55±0.27)mmol/L,具有统计学差异(P<0.05)。(2)MTP-493T/T基因型患者的TC和LDL-C较MTP-493G/T和MTP-493G/G基因型患者有升高趋势,HDL-C呈降低态势。(3)MTP-493T/T基因型患者经过治疗其血脂变化△TC、△LDL-C与MTP-493G/T、MTP-493G/G基因型患者相比明显减少,具有统计学差异(P<0.05)。结论 MTP-493位点G→T的单碱基变异影响2型糖尿病患者的血脂水平,同时,MTP-493基因型与辛伐他汀降低TC和LDL-C的幅度明显相关。
Objective To investigate the relationship between microsomal triglyceride transfer protein gene(MTP)and the effect of simvastatin on type 2 diabetes dyslipidemia.Methods Four hundred and twenty-eight patients of type 2 diabetes with hypercholesterolemia were treated with simvastatin 20 mg/d for 12 weeks.MTP-493G/T polymorphism was detected by PCR-RFLP and DNA sequence analysis.Results(1)TG and VLDL-C in patients with MTP-493T/T gene were significantly higher than that in MTP-493G/T gene and MTP-493G/G gene patients.The data were TG(2.89±1.83)mmol/L;(1.58±0.88)mmol/L;(1.68±1.25)mmol/L.VLDL-C(0.69±0.32)mmol/L;(0.52±0.26)mmol/L;(0.55±0.27)mmol/L(P<0.05).(2)The level of TC and LDL-C in patients with MTP-493T/T gene were little higher than that of MTP-493G/T gene and MTP-493G/G gene group.(3)The amount of △TC and △LDL-C in patients with MTP-493T/T gene were significantly decreased compared with either MTP-493G/T or MTP-493G/G carriers.Conclusions Single nucleotide mutation of G→T of MTP-493 genes has significant influence on dyslipidemia in type 2 diabetes.MTP-493 genotype has statistical correlation with the effect of simvastatin.
