骨折椎固定在胸腰椎骨折中的应用分析

Surgical Analyses of 38 Cases of Thoracolumbar Fracture and Dislocation Treated by Fixing the Pedicle Screws into Fractured Vertebrae

摘要 目的探讨骨折椎置钉治疗胸腰椎骨折脱位的疗效。方法2003年3月至2008年7月对38例胸腰椎骨折脱位患者均采用一期后路行椎弓根钉系统整复,其中单个椎体骨折27例,2个以上椎体骨折5例,合并脱位6例;所有的病例均使用骨折椎及上下椎置钉,手术均于伤后5~12d内完成。共完成骨折椎置97枚。结果全部病例均得到随访,平均13(3~17)个月。1例骨折椎未得到复位,仅椎间隙被过度撑开。术后骨折椎体复位达到84%,脱位获得良好的纠正。结论采用骨折椎置钉手术方式治疗胸腰椎骨折脱位可取得良好的复位效果,且近期并发症少。 Objective To explore the efficacy and surgical experience of the way of fixing the pedicle screws into the fractured vertebrae.Methods From March 2003 to July 2008,38 cases with thoracolum2bar fracture and dislocation including 27 cases of single vertebral fracture,5 cases of more than two vertebral fractures,6 cases complicated with dislocation were treated with pedicle screws. All the patients were operated within 5--12 days after trauma.Both fractured vertebra and it s adjacent upper and lower vertebra were implanted with pedicle screws.Fractured vertebrae were completed for 97nails.Results All cases were followed up from 3 to17 months,and the mean follow--up period was 13 months with a range from 3 to 17 months.One cases of vertebral fractures had not been reset,only intervertebral space to be excessively distrracted.Fractured vertebra reduction reached 84%,and vertebral dislocation received good correction.Conclusion For the treatment of thoracolumbar vertebral fracture,fixing the pedicle screws into the fractured vertebrae is feasible and can may produce good clinical results,and the early complications can be reduced.
