摘要
目的完善出院患者延续护理工作,有针对性地为患者提供帮助,提高患者遵医行为和自我护理能力,更好地为出院患者服务。方法通过收集特需病房400例出院患者延续护理资料,对其进行分析、总结,发现存在护士指导的健康教育内容欠个性化、预先告知患者不到位、经管医生参与率低等问题,并提出相应对策。结果延续护理工作质量提高,出院患者院外满意率由85.0%上升至98.0%,患者的再回访率明显提高。结论出院患者延续护理工作的不断改进,能有效提高出院患者生活质量,在护士和患者乃至家庭成员间建立有目的的互动,促进和维护患者健康,是一种由医院走向社会且可以节省人力资源的延伸访视形式,值得进一步深化和推广。
Objective To improve the quality of continued-nursing service for discharged patients and provide pertinence measures for them. And improve the compliance behaviors and self-care ability of patients,so as to provide better service for patients. Methods Through the analysis and summarization of the information about 400 cases of continued-nursing, some problems were found such as the contents of health education by charge nurse were not standard,the nurses did not tell the whole contents of the continued-nursing well,and the participation of the charge doctors was lower in the follow-up. Then some counter measures were proposed aimed at the problems. Results After taking the counter measures, the quality of continued-nursing service was improved and the patients＇ satisfaction rate raised from 85. 0% to 98. 0%, and the retention rate increased significantly. Conclusion Continued-nursing service for discharged patients could effectively improving the patients＇ life quality, establish interactive communication among nurses, patients and other family members, and promote health maintenance through constantly improving, which need to be further studied and generalized.
