水冷式双极射频纤维环成形术治疗盘源性腰痛 被引量:2

Effect of water-cooled dipolar radiofrequency annuloplasty on discogenic low back pain

摘要 目的探讨水冷式双极射频纤维环成形术治疗盘源性腰痛的临床效果。方法随机选取23例经椎间盘造影诊断为盘源性腰痛、经过正规保守治疗3个月无效的患者,采用水冷式双极射频纤维环成形术治疗,所有患者术后随访6个月。对患者术后第7天与6个月后的疗效、视觉模拟评分(VAS)、Oswestry功能评分(ODI)和阿片类药物使用量进行比较。结果术后第7天的优良率为87%(20/23);VAS由术前(7.6±0.7)分降至(3.1±0.9)分(P<0.01);ODI评分由术前(71.4±16.2)%降至(31.2±15.2)%(P<0.01);美施康定和曲马多的使用量分别由术前(35.0±10.3)mg和(300.0±19.6)mg降至(5.0±1.1)mg和(50.0±14.7)mg(P<0.05)。6个月后的优良率为78.2%(18/23);VAS和ODI评分分别为(3.3±1.0)分和(35.6±16.9)%,明显低于术前(P<0.01)。本组未见严重并发症发生。结论水冷式双极射频纤维环成形术操作简单、安全,是治疗盘源性腰痛的有效微创手术。 Objective To explore the clinical outcomes of water-cooled dipolar radiofrequency annuloplasty(WCDRA) in the treatment of dicogenic low back pain by. Methods Discogenic low back pain by discography was diagnosed in 23 patients anti treated with WCDRA. The patients were followed up for 6 months. The whole curative effect, visual analog scale(VAS) and oswestry disability index(ODI) scores were evaluated on the 7th postoperative day and at the end of 6-mounth follow up. The dosage of opioids used was recorded as well. Results On the 7th postoperative day,the excellent rate of treatment was 87%(20/23), VAS pain scores were decreased from (7.6±0.7) scores to (3.1±0.9)scores (P〈0.01),ODI decreased from(71.4±16.2)% to (31.2±15.2)% (P〈0. 01) ,and the dosages of opioids use expressed in morphine sulphate and tramadol were decreased from (35.0 ±10. 3) mg and (300. 0±19. 6) mg to (5.0±1.1) mg and (50. 0±14.7) mg (P〈0.05) ,respectively. At the end of 6-month follow-up, the cure rate was 78.2%(18/23),VAS scores and ODI were (3.3±1.0) scores and ( 35.6± 16.9)% (P〈0.01 ), respectively. No serious complications occurred. Conclusion The WCDRA is a rapid,efective,minimally invasive and safe procedure for the treatment of discogenic low back pain.
