摘要
目的:研究幽门螺杆菌(Helicobacter pylori,Hp)iceA基因型与胃十二指肠疾病的关系,旨在寻找内蒙古地区Hp iceA基因型的分布特点。方法:从164例胃十二指肠疾病(包括CSG 32例,CAG 70例,PUD32例,GC 30例)患者的胃黏膜活检组织中分离培养并经系统鉴定确认为Hp者,传代培养2 d扩增细菌。用标准方法提取Hp基因组DNA,进行PCR扩增检测Hp iceA基因的表达情况。iceA基因与疾病关系的分析采用χ2检验,P<0.05认为有统计学意义。结果:在164例临床分离的Hp菌株中,iceA基因的检出率为56.71%,iceA 1基因的检出率为39.63%(65/164),iceA 2基因的检出率为30.49%(50/164),iceA1的检出率高于iceA 2,差异有统计学意义(P<0.05)。在各疾病组中,iceA 1检出率分别为CSG 40.63%,CAG37.14%,PUD 43.75%,GC 40.00%,iceA 2检出率分别为CSG28.13%,CAG28.57%,PUD31.25%,GC33.33%,iceA 1和iceA 2基因在各组疾病中的检出率差异无统计学意义(P>0.05)。结论:Hp临床分离株的基因型以iceA 1为主,Hp感染后其iceA基因型不能预示感染的个体将更有可能发生何种胃十二指肠疾病,iceA基因不能作为Hp毒力强弱的指标。
Objective:To investigate the distribution of Helicobacter pylori (Hp) iceA gene in patients with gastriduodenal diseases. To contribute to searching for distrbution of Hp iceA gene in Inner Mongolia. Methods: A total of 164 patients with different gastric duodenal diseases, with culture- proven Hp infection were recruited in this study. Hp strains were transfer of culture for amplification bacteria. The Hp DNA was extracted using the standard method of phenol/chloroform. The iceA genotypes were determined by PCR reactions. The relationship between individual genotypes and the different gastric diseases was assessed with Chi - square ?(2 test, a (P〈 0.05) was defined as significant. Results: In all 164 patients, the prevalence of iceA was 56.71%, ieeA1 was 39.63 %, iceA2 was 30.49 % (P 〈 0.05). The iceA1 genotype was predominant in this study. The prevalence of iceA1 was CSG 40.63 %, CAG 37.14 %, PUD 43.75 %, GC 40.00 %, iceA 2 was CSG28.13 %, CAG 28.57 %, PUD 31.25 %, GC 33.33 %. There was no relation between the prevalence of iceA1 and iceA2 genotypes in gastric duodenal disease(P 〉 0.05). Conclusions:The iceA positive iceA1 genotype was predominant in this study. There was no correlation between iceA genotype and various gastroduodenal disease, iceA genotype was can not be considered markers which was more pathogenic.
