脑出血患者血肿周围脑组织水通道蛋白-4的表达变化 被引量:3

The Change of aquaporin-4 Expression in Perihematomal Tissue of Intracerebral Hemorrhage Patients

摘要 目的:探讨脑出血血肿周围脑组织水通道蛋白-4(AQP4)的表达规律及其与脑水肿形成的关系。方法:32例行颅内血肿清除术患者,按发病至手术时间不同分为<6 h、6~24 h、24~72 h和>72 h组。取术中获得的脑组织做病理标本,检测AQP4的表达变化,并观察其组织病理及超微结构变化。结果:<6 h组脑组织轻微水肿,AQP4表达呈弱阳性,6 h后水肿逐渐加重,AQP4表达开始增强,至24~72 h达高峰。此后,AQP4表达下降,水肿减轻。结论:脑出血后AQP4表达呈动态变化与脑组织水肿程度一致,提示AQP4在脑出血患者脑水肿的发生、发展中起着重要作用。 Aim: To investigate the relationship between the expression of aquaporin-4(AQP4) and cerebral edema formation in perihematomal tissue of hypertensive intracerebral hemorrhage patients. Methods: 32 patients with hypertensive intracerebral hemorrhage were selected and divided into 〈6 h, 6- 24 h, 24- 72 h and 〉72 h groups according to the time from the onset of illness to operation. Perihematomal brain tissues were obtained in the course of hematoma elimination. The change of aquaporin-4 expression was evaluated while the pathological and ultrastructural changes of the brain tissue were respectively observed. Results:The brain tissues within 6 hours group were slightly damaged and the expression of AQP-4 was slightly positive, severely damaged and gradually increased of the AQP-4 expression after 6 hours of intracerebral hemorrhage,and got to the maximum between 24-72 h. After 72 h of intracerebral hemorrhage, the expression of AQP4 was gradually decreased and the brain edema was improved. Conclusion:The dynamic change of aquaporin-4 expression can be observed in perihematomal tissue of intracerebral hemorrhage patients and is closely associated with the level of cerebral edema, suggesting that AQP-4 should play an important role in the course of the occurrence and development of the hemorrhagic cerebral edema.
