摘要
目的:研究稳定期慢性阻塞性肺疾病(COPD)患者使用无创通气治疗时的依从性和护理对策。方法:前瞻性分析影响我院接受双水平气道正压(BiPAP)通气治疗的52例稳定期COPD患者依从性的因素和提高患者依从性的护理策略。结果:怀疑疗效、无安全感、通气问题、气道问题、罩压皮损、胃肠胀气、操作麻烦等7个方面是影响患者依从性的常见原因,采取相应的心理治疗和技术改良可以明显提供患者的依从性。结论:影响患者BiPAP通气治疗依从性的因素是多方面的,我们可以采用心理治疗和技术改良的护理策略来提高患者的依从性。
Objective：To investigate the factors affecting the compilance of patients with chronic obstructive pulmonary disease （COPD） under the treatment of bilevel positive airway pressure（BiPAP） in the stabilization phase and the tactics improving the comipilance. Method：52 patients with COPD in the stabilization phase weres treated BiPAP in our department and their compilance were analysed prospectively. Results ：The frequent factors affecting the compilance of patients with COPD were suspicion of effect, no security, problems of ventilation, problems of air tract, skin damage from mask, puff of gastrontestinal tract and complex operation of machine. Psychological treatment and certain nursing technology could improve greatly the complicance of patients with COPD. Conclusions： The factors which affect the compilance patient with COPD under the treatment of BiPAP were various, and we could improve their compilance by psychological treatment and certain nursing technology.
