摘要
目的:探讨浆细胞性乳腺炎的临床诊断及手术治疗。方法:回顾性分析48例经病理确诊的浆细胞性乳腺炎患者的术前诊断及手术资料。结果:术前诊断为乳腺癌12例、导管内乳头状瘤8例、炎性包块7例、纤维腺瘤6例、乳腺结核3例、浆细胞性乳腺炎12例,误诊率75.0%。10例行局部肿块切除术,24例行乳腺区段切除术,5例行脓肿切开引流术,4例行窦道或瘘管切除术,5例行单纯乳房切除术。全部病例均痊愈出院,随访无复发。结论:本病术前极易误诊,术中应行冰冻切片病理检查及术后石蜡切片病理检查确诊;手术切除病灶是本病唯一彻底有效的治疗方法。
Objective To study the clinical diagnosis and surgical treatment of plasma cell mastitis. Methods The data of 48 patients plasma cell mastitis which were pathological diagnosed were retrospectively analyed. Results Before the operation 12 patients were diagnosed breast carcinoma, 8 were intraductal papilloma, 7 were inflammatory mass, 6 were fibroadenoma, 3 were mammary tuberculosis, and 12 were plasma cell mastitis. The misdiagnostic rate was 75. 0%. Ten received partial lumpectomy, 24 segmental mastectomy, 5 abscess incision drainage, 4 sinus tracts or fistulectomy, and 5 simple mastectomies. All recovered with no recurrence. Conclusion This disease is easy to be misdiagnosed before operation. Intraoperative frozen section and postoperative paraffin section should be done. Surgical resection is the only complete and effective treatment.
