摘要
目的探讨纳米羟基磷灰石/聚酰胺(n-HA/PA66)复合生物活性人工椎体在颈椎前路椎间融合治疗脊髓型颈椎病中的应用效果。方法自2003年12月至2007年8月应用纳米羟基磷灰石/聚酰胺(n-HA/PA66)复合生物活性人工椎体椎间融合结合钛板颈前路手术治疗26例脊髓型颈椎病患者,并成功随访24例,术后随访观察神经功能恢复情况及融合效果。结果随访4~36个月,平均20.5个月,全部患者术后未见任何过敏或毒性反应。JOA评分从术前(9.3±2.1)分提高到术后(15.2±3.2)分,差异有统计学意义(P=0.0232)。术后X线片未见人工椎体塌陷、移位或脱入椎管,无钛板或螺钉松动及折断,无明显颈椎生理曲度改变,术后所有病例均获得椎间融合,融合时间(8.6±3.4)个月。结论纳米羟基磷灰石/聚酰胺(n-HA/PA66)复合生物活性人工椎体具有良好的生物相容性和生物安全性,植入融合率高,牢固可靠,联合颈椎前路锁定钢板系统可以有效地进行颈椎前路融合和维持颈椎生理曲度。
Objective To evaluate the effect of artificial vertebral body of the biomimetic n-HA/PA66 composites in the anterior cervical fusion for cervical spondylosis myelopathy with anterior instrumentation. Methods From December of 2003 to August of 2007,26 cases of cervical spondylosis myelopathy were treated by anterior semivertebra decompression,intervertebral fusion with artificial vertebral body of the biomimetic n-HA/PA66 composites, fixation with AO plate. Twenty-four cases were followed up. Nerve functional restoration and intervertebral fusion were observed. Results The follow-up was ranged from 4 months to 36 months,with an average 20.5 months. In all patients,the statistical analysis revealed the JOA scores of 9.3±2.1 and 15.2±3.2 before operation and at the final examination. The X-ray film demonstrated that the height of intervertibal space has restored and there was no loosening of implant and shift of artificial vertebral body of the biomimetic n-HA/PA66 composites graft. All cases were gained solid fusion and the fusion period was (8.6± 3.4) months. Conclusion Artificial vertebral body of the biomimetic n- HA/PA66 composites has a good biocompatibility, high security, high rate of bone graft fusion and firm fixation. When combined with cervical anterior locking plate,it can obtain a satisfactory clinical results.
