超声乳化人工晶体植入术后早期眼压变化 被引量:2

Clinical research on early changes of intraocular pressure after phacoemulsification and intraocular lens implantation

摘要 目的探讨超声乳化人工晶体植入术后24h内眼压变化及术中使用缩瞳剂对减少术后早期高眼压的影响。方法对43例(43眼)行超声乳化人工晶体植入术的老年性白内障患者术前和术后2、4、6、8、10、24、48h的眼压进行观察,比较术中使用缩瞳剂的患者和未使用缩瞳剂的患者术后各时间点的眼压变化情况。结果43眼术后2~10h的眼压较术前明显升高(P〈0.01),术后4~10h的平均眼压高于21mmHg;术后24h的平均眼压恢复至正常水平,但仍比术前高;术后48h的眼压与术前无统计学差异(P〉0.05)。术中使用缩瞳剂和未使用缩瞳剂的患者术后各时间点的眼压无统计学差异(P〉0.05),但未使用缩瞳剂的患者中有5例(5/28,17.9%)术后出现30mmHg以上的高眼压,使用缩瞳剂的患者中无一例眼压高于30mmHg。结论超声乳化人工晶体植入术后早期眼压有不同程度的升高,个别患者眼压高于30mmHg。术中使用缩瞳剂不能完全预防术后眼压升高,但能减少出现30mmHg以上的过高眼压。 Objective To analyze the increased intraocular pressure (IOP) on the same day after uneventful phacoemulsification (Phaco) and intraocular lens (IOL) implantation. The effect of carbachol on the IOP was also analyzed. Methods The IOP of 43 age-related cataract patients (eyes) who accepted uneventful cataract Phaco and IOL implantation was observed and analyzed before operation and 2, 4, 6, 8, ll), 24 and 48 hours postoperatively. Carbachol was used in 15 patients (eyes). Results IOP were higher at 2, 4, 6, 8 and 10 hours postoperatively than the preoperative one (P〈0.01). IOP at 4, 6, 8 and 11) hours after operation were higher than 21 mmHg. IOP 24 hour after operation was in normal range, but was still higher than the preoperative one. Fourty-eight hours after operation the IOP was not significantly different from the preoperative one (P〉0. 05 ). Conclusions Elevation of IOP occurs on the same day after uneventful Phaco and IOL implantation, with a peak higher than 30 mmHg in some patients. Carbachol is unable to prevent IOP increase, but can decrease the IOP peak.
