Application of DHS for the treatment of intertrochanteric fractures with little invasion

摘要 目的探讨采用动力髋螺钉(DHS)微创治疗股骨转子间骨折的疗效。方法回顾性分析68例股骨转子间骨折患者,按EVANS分类:I型8例,Ⅱ型10例,Ⅲ型30例,IV型14例,V型6例。所有患者采用闭合复位、微创小切口下DHS内固定治疗。分析手术时间、术中出血量,随访骨折愈合情况,并采用Parker评分评估髋关节功能。结果68例患者平均切口长度5.1±1.0cm,平均出血量约160±20mL,平均手术时间48±10min。术后平均随访26个月,Parker评分优良率94.1%。结论小切口下动力髋螺钉(DHS)微创治疗股骨转子间骨折手术操作较简便,创伤小、出血少、并发症发生率低,具有良好优越性。 [Objective] To explore the treatment effect of DHS internal fixation in treating intertrochanteric fractures with little invasion. [Methods] Sixty-eight patients with intertrochanteric fractures were retrospectively analyzed. According to Evans classification, there were 8 cases of type Ⅰ, 10 cases of type Ⅱ, 30 cases of type Ⅲ, 14 cases of type Ⅳ and 6 cases of type V. All patients were treated with closed reduction and DHS fixation under little invasion. We analyzed the length of incision, the operation time and volume of blood loss, and followed up the condition of fracture union and evaluated hip function with Parker score. [Results] In all 68 patients, the average length of incision was 6.1±1.0 cm, the average volume of blood loss was 160±20 mL, the average operation time was 48±10 rain. The post operation follow time was 26 months, the fineness rate of Parker score was 94.1%. [Conclusions] DHS fixation with little invasion is a good choice to treat intertrochanteric fractures, which have some merits, including simple manipulation, little operation invasion, less blood loss, and low incidence of complication.
