摘要
目的探讨螺旋 CT 平扫对闭合性肠及肠系膜损伤的诊断价值。方法对51例经手术、临床证实且术前 CT 资料完整的闭合性肠及肠系膜损伤患者进行回顾性分析。所有病例均未口服造影剂及增强扫描。结果51例中,肠壁增厚27例,肠壁血肿5例,腹腔或腹膜后游离气体15例,肠系膜血肿11例,肠系膜脂肪渗出19例,肠袢间或肠系膜周围积液28例,腹腔游离积液31 例,肠管扩张积液3例及胃扩张3例等。CT 诊断符合率为90.2%。讨论螺旋 CT 平扫能快速、准确地诊断闭合性肠及肠系膜损伤.对治疗方案的选择有重要的临床价值。
Objective To investigate the diagnostic value of spiral CT in the blunt bowel and mesenteric trauma. Methods The CT findings of 51 cases with blunt bowel and mesenteric trauma proved by operation or clinical were reviewed. All the patients were performed without using oral and intravenous contrast material. Results Of the 51 case, CT findings were as followings： bowel wall thickening （n=27）, bowel wall hematoma （n=5）, intraperitoneal or retroperitoneal flee air （n=15）, mesenteric hematoma （n=11）, infiltration of mesenteric fat （n=19）, surrounding bowel and mesenteric fluid （n=28）, intraperitoneal free fluid （n=31）, intestinal enlargement with fluid collections （n=3） and stomach enlargement （n=3）. The diagnostic compatibility rate of CT before surgery was 90.2%. Conclusion Spiral CT scan is fast, sensitive in diagnosis of blunt bowel and mesenteric trauma, and of important clinical value in the decision to treatment.
