摘要
目的:探讨甲状腺低密度结节(甲状腺癌、甲状腺腺瘤与结节性甲状腺肿)螺旋CT动态增强扫描CT表现的差异,总结出有鉴别诊断价值的CT征象。方法:回顾分析7例甲状腺癌、5例甲状腺腺瘤与20例结节性甲状腺肿,均经手术或穿刺病理证实。所有病例均采用螺旋CT动态增强扫描,绘制三种病变的时间密度曲线,结合病理改变,总结三种病变的CT征象及时间密度曲线的差异。结果:甲状腺癌密度不均匀4例,边界不清5例,病变内见钙化1例,囊变坏死3例。注药后不均匀增强,时间密度曲线为慢升慢降型。甲状腺腺瘤密度均匀4例,边界清晰4例,囊变坏死1例。注药后轻度均匀强化,时间密度曲线为平稳型。结节性甲状腺肿密度一般均匀18例,边界欠清晰12例,内见坏死2例。注药后均匀明显强化15例,时间密度曲线为速升速降型。结论:螺旋CT动态增强扫描可对甲状腺低密度结节做出鉴别诊断。
Objective：To assess the diagnostic value of spiral CT dynamic-enhanced scanning in thyroid low-density node （thy- roid cancer, thyroid adenoma and nodular goiter）. Methods： Seven patients with thyroid cancer, 5 patients with thyroid adenoma and 20 patients with nodular goiter were given dynamic-enhanced scanning. Time-density curve and pathological changes were studied and CT findings of the three diseases were summarized. Results： Thyroid cancer often showed asymmetric density （4 cases）, blurry boundary （5 cases） and its contrast enhanced density rise and down slowly. Of the 7 patients with thyroid cancer, l was found calcification and 3 were found necrotic cyst. Thyroid adenoma often showed symmetrical density （4 cases）, clear boundary （4 cases） and its enhanced density rose smoothly with the time delay. Of the 5 patients with thyroid adenoma, 1 was found necrotic cyst. Nodular goiter showssymmetrical density （18 cases） and unclear boundary （12 cases） and its contrast enhanced density rose quickly and down quickly. Of the 20 patients with nodular goiter, 2 were found necrotic cyst. Conclusion： By using time-density curve in dynamic-enhanced scanning, differential diagnosis of thyroid cancer, thyroid adenoma and nodular soiter can come true.
