摘要
学术背景:近些年来胸段置入椎弓根钉技术广泛普及,使得椎弓根钉系统在治疗特发性脊柱侧凸中起到越来越关键的作用。目的:了解椎弓根钉系统在治疗特发性脊柱侧凸的应用。检索策略:由该论文的研究人员应用计算机检索Pubmed数据库1997-01/2007-01的相关文献,检索词为"pedicle screw,IS"并限定文章语言种类为English。同时计算机检索中国期刊全文数据库1997-01/2007-01的相关文献,检索词"椎弓根钉,特发性脊柱侧凸",并限定文章语言种类为中文。共检索到116篇文献,对资料进行初审,纳入标准:椎弓根钉系统的特点和治疗特发性脊柱侧凸相关的文献。排除标准:重复研究,Meta分析。文献评价:文献的来源主要是对椎弓根钉系统治疗特发性脊柱侧凸的研究。所选用的30篇文献中,3篇为综述,其余均为临床或基础实验研究。资料综合:特发性脊柱侧凸多为三维平面的复杂畸形,现代脊柱侧凸治疗的目的是达到脊柱三维畸形矫正。椎弓根螺钉提供了贯穿脊柱三柱的强大控制力,并把矫形棒和脊椎连接起来,不仅作用力强、牢固可靠,而且还同时具有对脊柱的去旋转、节段撑开、节段加压和水平矫正等功能,可更有效的发挥对脊柱的三维矫正作用。结论:椎弓根钉系统治疗特发性脊柱侧凸是安全可靠的,此外椎弓根钉系统提供了确实的三维矫形,并能够保留部分活动能力。
BACKGROUND: Thoracic pedicle screw technique has been widely used in recent years, so the pedicle screw system plays a more key role in the treatment of idiopathic scoliosis (IS). OBJECTIVE: To review the application of pedicle screw system in the treatment of IS.
RETRIEVAL STRATEGY: A computer-based online search of Pubmed database was undertaken for pertinent literatures related to pedicle screw and IS published from January 1997 to January 2007 by using the keywords of "pedicle screw, IS" in English, and Chinese Journal Full-text Database was also searched for the related articles between January 1997 and January 2007 with the same key words in Chinese. Totally 116 articles were collected after the first trial. Inclusive criteria: the article referring to the feature of pedicle screw system and the treatment of IS. Exclusive criteria: repetitive research and Meta analysis. LITERATURE EVALUATION: The literatures were mainly related to the pedicle screw systems in the treatment of IS. Thirty literatures were selected, including 3 were review articles while the others were clinical or basic experimental studies. DATA SYNTHESIS: Three-dimensional complex deformation is chief for IS, therefore the treatment of IS is aimed to correct the three-dimensional deformation. Pedicle screw fixation has been proved to enable true segmental control and greater connection between rods and spine reliably. Additionally it is available for three-dimensional correction through derotation, segmental opening, segmental pressing and orientation.
CONCLUSION: Pedicle screw system is a safe procedure for treating adolescent IS. Moreover, the treatment by pedicle screw fixation offers three-dimensional correction and preserves the motion segments.
