摘要
目的通过对北京地区16个常见病种的病案首页数据进行分析，找出北京地区患者常见病种住院医疗费用的主要影响因素，为有效控制医疗费用的不合理增长提供政策性建议。方法以北京市22所医院2003年和2004年两年16个病种全部出院病例为研究样本，采用单因素分析和多元逐步回归分析，对可能的影响因素进行筛选，以确定影响患者住院费用的主要影响因素。结果影响患者住院费用的因素包括住院天数、医疗付费方式、医院等级、有无手术、有无并发症／合并症、年龄、入院情况、有无抢救、转归等。结论影响住院费用的因素是多方面的，既包括各种社会学因素，也包括各种临床因素。住院费用控制是一项系统工程，必须采取综合性措施，才能实现政府、患者、医院各方利益的均衡发展。
Objective To explore the main factors influencing the hospitalization expenses in Beijing by analyzing the first page data of medical records of 16 common diseases and to make policy suggestions for controlling the irrational increase of medical expenses. Methods The data of 16 common diseases in discharged patients of 22 hospitals in Beijing during 2003-2004 were collected, and single factor analysis and multiple stepwise regression analysis were used to screen and identify the principle factors influencing the hospitalization expenses. Results The factors influencing the hospitalization expenses included hospital stay, payment mode, grade of hospital, procedure, complication or comorbidity, age, status at admission, rescue and prognosis. Conclusion Multiple factors can influence the hospitalization expenses, including both the sociological factors and clinical ones. The hospitalization expense control is a systematic project, and comprehensive measures must be taken to achieve the balanced development of the interests of government, patients and hospitals.
