摘要
目的观察超激光(SL)照射星状神经节(SG)对乳腺癌患者围术期促肾上腺皮质激素(ACTH)、皮质醇(Cor)及体液免疫的影响。方法择期乳腺癌改良根治术女性息者40例,随机分成两组,对照组(Ⅰ组,n=20),照射组(Ⅱ组,n=20)。照射组在麻醉诱导前用超激光治疗仪(Super Lizer HA-550)先后照射两侧SG,功率60%～80%,照2s,停2s,每侧各10 min。Ⅰ组不启动治疗仪,余同Ⅱ组。观察入室(T1)、切皮(T2)和术毕30 min(T3)时ACTH、皮质醇和IgG、IgA、IgM的变化。结果与T1比较。Ⅰ组T3时ACTH和T2、T3时皮质醇均显著升高(P<0.05,P<0.01);Ⅱ组仅T3时皮质醇明显升高(P<0.05),Ⅰ组T3时ACTH和皮质醇明显高于Ⅱ组(P<0.05)。与T1比较,T2、T3时两组IgG、IgA、IgM均有不同程度下降(P<0.05,P<0.01),T3时Ⅱ组IgG明显高于Ⅰ组(P<0.05),T2、T3时Ⅱ组明显高于Ⅰ组(P<0.05)。结论术前超激光照射星状神经节可减轻乳腺癌改良根治术患者体液免疫的抑制程度,其机制可能与减轻麻醉和手术所致应激反应有关。
Objective To observe the effects of Super Lizer（SL） irradiation to stellate ganglion （SG） on perioperative ACTH,cortisol and humoral immunity for patients received modified radical mastectomy （MRM）. Methods Forty femal ASA I-Ⅱ patients scheduled for sdective MRM were randomly divided into two groups： control group （ group I , n =20） and irradiation group （group Ⅱ, n=20）. Before anesthesia the patients in group Ⅱ were received SL irradiation to both side SG with 60%-80% output, switching on for 2 seconds and off for 2 seconds over a period of 10 min each side. The method of group I was the same as group II but no irradiation. Blood samples were taken after entering operating room （T1）, cutting skin（T2） and 30min after operation（T3） for determination of ACTH,cortisol and IgG,IgA,IgM. Results Compared with T1 ,ACTH at T3 and cortisol at T2,T3 in group I were higher significantly,while only cortisol at T3 in group II was increased significantly. ACTH and cortisol at T3 were significantly increased than that of group II. Compared with T1, IgG, IgA, IgM at T2 and T3 in group II were higher significantly than that of group I. Conclusion SL irradiation to SG before operation can reduce inhibition of perioperative humoral immunologic function induced by stress.
