止血带引起肢体缺血再灌注损伤与七叶皂苷钠的预处理 被引量:9

Protective effects of preconditioning with sodium aescinate on ischemia/reperfusion injury of the limbs induced by tourniquet

摘要 目的:七叶皂苷钠能够清除机体内自由基,起到保护血管壁的作用,实验拟进一步观察七叶皂苷钠预处理对止血带引起的肢体缺血再灌注损伤的保护作用。方法:选取南京中医药大学附属医院2006-08/2007-02期间择期骨科下肢手术患者40例。分为对照组20例,给药组20例。给药组在上止血带前30min静脉滴注含有5mg七叶皂苷钠的注射液100mL;对照组在相同时间点给予等量生理盐水。在上止血带前以及松止血带后5,10,20min抽取静脉血4mL分别测量血乳酸、超氧化物歧化酶、丙二醛和一氧化氮的含量变化,并观察患者在4个时间点的血压及脉搏的变化。结果:40例患者均进入结果分析。①与放气前比较,对照组舒张压在放气后5,10min均下降(P<0.05),乳酸在放气后5,10min升高(P<0.05)。②与放气前比较,两组乳酸水平在放气后5min,10min,20min均升高(P<0.05)。③与对照组比较,给药组超氧化物歧化酶值在放气后5,10,20min均升高(P<0.05),舒张压在放气后5min升高(P<0.05),乳酸在放气后10min升高(P<0.05),收缩压、丙二醛、一氧化氮值均无变化。结论:七叶皂苷钠预处理对止血带引起的肢体缺血再灌注损伤有一定的保护作用,它可能是通过提高超氧化物歧化酶的活性来实现,对乳酸,丙二醛和一氧化氮变化无明显影响。 AIM:Sodium aescinate can protect the vessel wall while clearing free radical in body. This study was designed to approach the protective effects of preconditioning with sodium aescinate on ischemia/reperfusion injury of the limbs induced by tourniquet. METHODS: Forty patients undergoing elective lower limb operation were selected from the Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from August 2006 to February 2007. They were randomly divided into 2 groups: 20 in treated group received sodium aescinate (5 mg/100 mL) by intervenous drop infusion, while 20 cases in control group received equal dose of Sodium Chloride at 30 minutes before fastening tourniquet. Blood samples (4 mL) were obtained before fastening tourniquet and at 5, 10, 20 minutes after release of tourniquet to measure superoxide dismutase (SOD), malondialdehyde (MDA), lactic acid (Lac) and nitrogen monoxidum (NO). Meanwhile the blood pressure and heart rate were recorded in two groups. RESULTS: All of 40 patients were involved in the result analysis.①In the control group, diastolic blood pressure after 5 and 10 minutes of release was significantly less than that before (P 〈 0.05), and Lac was significantly higher than that before (P 〈 0.05).②Lac after 5, 10, 20 minutes of release in two groups was significantly higher than that before (P 〈 0.05). ③Compared with the control group, SOD after 5, 10, 20 minutes of release was significantly higher in the treated group (P 〈 0.05), diastolic blood pressure increased after 5 minutes (P 〈 0.05), and Lac increased after 10 minutes (P 〈 0.05). There was no difference in systolic blood pressure, MDA and NO value between the two groups. CONCLUSION: Pretreatment with sodium aescinate has protective effects against ischemia/reperfusion injury of the limbs induced by tourniquet. There is no influence with Lac, MDA and NO. Perhaps it implements the protection through elevating the activity of SOD.
