Implementing Clinical Pathway Management to Construct Harmonious Doctor-patient Relation

摘要 目的:通过对单病种乳腺癌改良根治术等10个单病种临床路径与成本控制绩效分析,评价患者的满意效果。方法:收集整理2005年10月至2007年3月10个单病种的统计信息,以其手术并发症发生率、平均住院天数、平均住院费用、患者满意率等作为评价指标,将实验组(应用临床路径和成本控制组)和对照组(未应用临床路径和成本控制组)进行对比分析。结果:应用临床路径和成本控制的患者与未应用这一方式的患者之间在手术并发症发生率、平均住院时间、平均住院费用、患者满意率方面有统计学显著差异。结论:单病种临床路径和成本控制模式应用于临床管理,可以明显促进CQI(Continuous Quali-ty Improvement),缩短平均住院时间,降低住院费用,提高患者满意率,并从根本上改善医患关系。 Objective: To evaluate patient satisfaction conditions and the implementation effect of clinical pathway (CP) and cost control ( CC ) of 10 single - disease entities. Methods: After collecting the information of the hospi-talized patients after the surgical therapies from October 2005 to March 2007, we analyzed the effect of CP and CC with the evaluation indexes of average length of stay and the hospitalization charge. Results : There was significant difference in CQI, the average length of stay, the hospitalization charge, and patient satisfaction rate between the CP- CC groups and the traditional groups. Conclusion: The method of clinical pathway and cost control can markedly improve medical quality and patient satisfaction rate, decrease the average length of stay and the hospitali-zation charges, and fundamentally improve the doctor-patient relation.
