摘要
目的比较半髋关节置换术与全髋关节置换术治疗老年完全移位股骨颈骨折的疗效。方法将1996年5月~2002年12月我院治疗的67例55岁以上有移位股骨颈骨折患者分为两组:A组行半髋关节置换术,B组行全髋关节置换术。随访12—70个月,平均62.9个月。结果按北京市髋关节置换术座谈会议标准,A组满意率91.43%;B组满意率90.62%,两组间没有明显差异。结论半髋关节置换术与全髋关节置换术是治疗老年移位股骨颈骨折的有效方法,能提高患者的生活质量。半髋关节置换术可优先选择。
Objective To compare the effects of hemiarthoplasty and total hip arthroplasty(THA) in treatment of the displaced fractures of femoral neck in the elderly. Methods A restrospective study was carried out in 67 cases of femoral neck fractures treated from May 1996 to December 2002. Sixty-seven elderly patients of over 55 years old with displaced femoral neck fracture ( Garden Type Ⅲ - Ⅳ ) were operated on with hemiarthoplasty( group A) or total hip arthoplastry( group B). The follow-up period ranged from 12 months to 72 months (mean 62.9 months ). The clinical results,the complication and rate of revision were compared. Results There were 32 excellent or good cases( 91.43% ) in hemiarthoplasty group and 29 excellent or good cases ( 90.62% ) in THA group. Radiological examinations showed that 33 cases had perfect acetabula,2 cases acetabular osteophytes and only 1 cases had stem sinking in group A. There was no significant difference in the rates of postoperative medical complication between A and B group. Conclusion Acetabular osteophytes is related to weight bearing,while acetabular erosion is possibly due to unsuitable prostheses selection. Both hemiarthoplasty and THA are reasonable choices for treatment of displaced fracture of femoral neck in elderly. Hemiarthoplasty is recommended for elderly patients.
