摘要
目的探讨低浓度(0.25%)5-氟尿嘧啶(5-FU)与糖皮质激素混合注射治疗胸骨前瘢痕疙瘩的效果。方法将本组的98例患者(共有167个胸骨前瘢痕疙瘩,病史1～42年)分成A、B、C三组,均行瘢痕疙瘩全层注射。对A组的22例患者行单纯5-FU(2.5%5-FU0.6 ml+2%利多卡因5.4 ml)注射,对B组的22例患者行单纯5-FU和糖皮质激素(1%曲安奈德5 ml+2%利多卡因1 ml)交替注射,对C组的54例患者行5-FU与糖皮质激素的混合液注射(2.5%5-FU 0.6 ml+1%曲安奈德5 ml+ 2%利多卡因0.4 ml)。在瘢痕疙瘩完全萎缩后,逐渐降低药物浓度,并延长注射间隔期。治疗次数为6～22次,治疗3～36个月,随访6～26个月。结果局部注射低浓度5～FU,能够减轻瘢痕的充血程度和痛痒症状(有效率均为95.5%),但只有在与糖皮质激素交替或联合使用后,才能促使瘢痕明显萎缩(P<0.05),而联合使用比交替使用更为有效(P<0.05)。结论低浓度5-FU与糖皮质激素联合治疗胸骨前瘢痕疙瘩,具有较良好的疗效。
Objective To examine the efficacy of intralesional injection of 5 - fluorouracil in low concentration（0.25 %） mixed with corticosteroid for the treatment of keloids in the sternal area. Methods A total of 98 patients with a total of 167 keloids in the sternal area were divided into A, B and C groups. All lesions were treated at 2 weeks intervals. Keloids in group A were treated with intralesional injection of 0.25 % 5 - fluorouracil （2.5% 5- fluorouracil 0.6 ml ＋ 2% lidocaine 5.4 ml）. Keloids in group B were treated with 0.25 % 5- fluorouracil and 1% corticosteroid by turns. Keloids in group C were treated with a mixture of 0.25 % 5 - fluorouracil and corticosteroid （ 2.5 % 5 - FU 0.6 ml ＋ 1% corticosteroid 5 ml ＋ 2 % lidocaine 0.4 ml）. After keloids became flattened, drug concentration was decreased and intervals of drug injection were prolonged. The length of therapy varied from 3 to 36 months and patients had followup for 6-26 months. Results Hyperemia and gargalesthesia of keloids in group A were improved slightly（95.5% effective rate）, but atrophy of keloids was marked in groups B and C, and the efficacy in group C was better than that in the group B（P 〈 0.05 ）. Conclusion Intralesional injection of 5 - fluorouracil in low concentration mixed with corticosteroid can achieve an effective therapeutic result for keloids in the sternal area.
