高压静脉注射造影剂药液外渗与注射部位相关性研究 被引量:55

摘要 目的寻求高压静脉注射造影剂外渗发生率较低的注射部位,以减少渗出的发生率。方法将穿刺部位分为肘正中静脉组(A)、前臂头静脉组(B)、前臂贵要静脉组(C)、前臂掌侧静脉组(D)和手背静脉组(E)五组。静脉穿刺针头与皮肤呈60°角进皮肤后直接进入血管,并血管内潜行6-10 mm,注射速度2.5-3ml/s。结果各组局部渗出发生率之间差异有统计学意义(P≤0.010);A组(0.31%)、B组(0.50%)和C组(0.78%)的局部渗出发生率差异无统计学意义;D组(1.38%)和E组(2.3%)的局部渗出率差异也无统计学意义。A组、B组和C组注射造影剂渗出率较低。结论上肢注射部位血管直径越大、侧支血液分流量越大、渗出率越低。 Objective To seek a less frequencies of contrast material extravasation at the injection site during contrast material delivered into veins with an automated power injector. Methods Five groups (A: median cubital v, B: Cephalic v, C: basilic v, D: veins of the anterior cubital region, E: veins of the dorsum of hand)were divided according to the site of injection of contrast media. Pinhead with skin shaped at 60° clamp when venipuncture was implemented and the pinhead penetrated into skin immediately intoveins and followed the veins 6- 10 mm. Contrast material was injected at rates 2.5-3 ml/sec. Results Extravasation rates at the injection sites were significantly differ- ent among the five groups (P≤0. 010) . Extravasation rates weren't different significantly among group A (0.31%) , B (0. 50%) and C (0. 78%), and weren't different significantly between group D (1. 38%) and E (2.3 % ) . Extravasation rates of group of A, B and C were significantly lower than group D and E. Conclusion The thicker veins and more blood diversion of injection of contrast material at upper limbs, the lower extravasation rate.
