摘要
目的探讨重型颅脑损伤性高血糖患者血浆肿瘤坏死因子-α(TNF-α)与胰岛β-细胞功能不全和胰岛素抵抗之间的关系。方法观察我院近年来50例重型颅脑损伤性高血糖患者血浆TNF-α、稳态模式评估法胰岛素分泌指数(HOMA-β)、糖负荷30min后胰岛素增量和血糖增量比值(ΔINS30/ΔGLU30)和稳态模式评估法胰岛素抵抗指数(HOMA-IR),并分析血浆TNF-α与HOMA-β、ΔINS30/ΔGLU30和HOMA-IR相关性。结果重型颅脑损伤性高血糖患者血浆TNF-α、HOMA-IR均显著高于正常对照组(P<0.01),HOMA-β和ΔINS30/ΔGLU30均显著低于正常对照组(P<0.01)。重型颅脑损伤性高血糖患者血浆TNF-α与HOMA-β和ΔINS30/ΔGLU30显著负相关、与HOMA-IR显著正相关(P<0.01)。结论重型颅脑损伤性高血糖患者存在胰岛β-细胞功能不全和胰岛素抵抗,血浆TNF-α在创伤后糖代谢障碍中可能发挥重要作用。
Objeαive To study the relationship of the plasma tumor necrosis faαor α (TNF-α) with the dysfunαions of islet β-cell and insulin resistance in hyperglycemia patients caused by severe brain injury. Methods The levels of the plasma TNF-α, the index of the insulin secretion of islet β-cell assessed by homeostasis model assessment (HOMA-β), AINS30/AGLU30 and the index of the insulin resistance (IR) assessed by homeostasis model assessment (HOMA-IR) in 50 cases of hyperglycemia patients caused by severe brain injury were observed, and the remarkable correlation between the plasma TNF-α and the index of HOMA-β, AINS30/AGLU30 and HOMA-IR in hyperglycemia patients caused by severe brain injury were analyzed. Results The plasma TNF-α and the HOMA-IR of the hyperglycemia patients caused by severe brain injury were higher than that of the control group (P 〈0.01 ), and the HOMA-β and AINS30/AGLU30 in hyperglycemia patients caused by severe brain injury were lower than that of control group ( P 〈 0.01 ). The plasma TNF-α was significantly reversely related to the index of HOMA-β and AINS30/AGLU3o, and significantly direαly correlated to the index of HOMA-IR in hyperglycemia patients caused by severe brain injury (P 〈 0.01 ). Conclusion The hyperglycemia patients caused by severe brain injury present dysfunαion of islet β-cell and higher degree of insulin resistance. The plasma TNF-α may play an important role on the dysfunαion of glucometabolism after brain injury.
