Different Concentrations of Trisodium Citrate for Anticoagulation in Patients Receiving Continuous Renal Replacement Therapy:a Comparative Study

摘要 目的探讨不同浓度的枸橼酸钠抗凝在连续性肾替代治疗中的应用效果。方法选择94例急慢肾功能衰竭行连续性肾替代治疗(continuous renal replacement therapy,CRRT)的患者,将患者随机分为观察组(48例)和对照组(46例),观察组给予低浓度枸橼酸钠抗凝,对照组给予高浓度枸橼酸钠抗凝。结果透析过程中患者生命体征平稳;所有患者均无抽搐、痉挛等明显低钙血症的表现;对照组中有16例患者的血清钠>150 mmol/L,观察组中未发生高血钠,两者比较有统计学意义(P<0.05)。结论CRRT患者使用低浓度的枸橼酸钠置换液,抗凝效果明显,不良反应少,易于控制,是一种安全有效的实施方案。 Objective To study the outcomes of different concentrations of trisodium citrate for anticoagulation in patients receiving continuous renal replacement therapy (CRRT). Methods Ninety-four patients receiving CRRT due to acute or chronic renal failure were divided into 2 groups: observation group( n =48) and control group( n = 46). Patients in the observation group were given low concentration of trisodium citrate for anticoagulation and patients in the control group were given higher concentration of trisodium citrate for anticoagulation. Results The vital signs of all patients were stable during the dialysis and none of them had muscular spasms or convulsion. Sixteen patients in the control group developed hypernatremia (sodium〉 150 mmol/L), which was not found in the observation group( P 〈0.05). Conclusion Low concentration of regional citrate for anticoagulation is safe, effective and easy-to-perform for patients receiving CRRT,with less adverse reaction.
