机械刺激对C型子宫内膜种植窗期组织形态学的影响 被引量:30

The effect of mechanical stimulation to endometrial morphology of pattern C during the implantation window

摘要 目的探讨机械刺激对C型子宫内膜种植窗期组织形态学的影响。方法选择自然周期尿LH(++)日子宫内膜呈C型改变的不孕妇女20例为研究组,随机分成两组,各组10例。结果不孕妇女种植窗期子宫内膜的组织结构形态学指标:腺体面积、腺体周长、腺体最大直径、腺腔面积、腺腔周长、腺腔最大直径、腺上皮细胞总面积、腺上皮平均高度以及子宫内膜组织结构构成比指标:腺体面积(%)、腺腔面积(%)、腺上皮细胞总面积(%)、间质面积(%)和腺/间比值在对照组较刺激组低,比较其差异有显著性(P<0.05)。结论晚卵泡期C型子宫内膜在种植窗期存在发育障碍,光镜下表现为内膜腺体发育迟缓,分泌不良。机械刺激可促进C型子宫内膜在种植窗期腺体发育,腺体表现顶浆分泌旺盛。机械刺激对于改善C型子宫内膜容受性是安全,有效的,临床具有一定可行性。 Objective To analyse the effect of mechanical stimulation to endometrial morphology of pattern C during the implantation window. Methods To choose 20 infertility women who exhibit a homogeneous pattern C of the endometrium on the peak day urine LH ( + ) of the natural cycle, all patients were divided into two groups randomly, each group are 10 patients. Results It was a signicant difference that the glandular area, glandular girth, maximal gland diameter, lumen of gland area, lumen of gland girth, lumen of gland maximum diameter, glandular epithelial cell centre line average, glandular epithelial cell total area and endometrium biopsies constituent ratio such as glandular area, lumen of gland area ( % ), glandular epithelial cell total area (%), stromal area (%), glandular area/stromal area between Control group and Stimulative group (P 〈 0. 05) . Conclusion Endometrium of pattern C was developmental disturbarce during window of the implantation at follicular phase, appeared epithelial gland of endometrium developed retardation and secertive poor by light microscope. Tow group have nothing to do with E2, P level in sernrn. Epithelial gland of endometrium were improved by mechanical stimulation during window of the implantation, Appeared epithelial gland secretive rich. That was safe and valid and feasibile method to improve endometrial receptivity by mechanical stimulation.
