幽门螺杆菌与原发性肝内胆管结石的关系 被引量:8

Pathogenic mechanism of helicobacter pylori in primary intrahepatic stones

摘要 目的研究幽门螺杆菌(helicobacter pylori,Hp)与原发性肝内胆管结石(primary intrahepatic stones,PIS)之间的关系。方法采用PCR法检测胆汁及肝内胆管黏膜组织中Hp—DNA;采用改良fishman法检测β-葡萄糖醛酸苷酶(β-Glucuronidase,β-G)。结果37例PIS患者胆汁及胆管黏膜行Hp—DNAPCR检测,总阳性数为26例(70.27%),而对照组胆汁中Hp—DNA仅1例阳性,Hp感染率两组之间比较差异有统计学意义(P〈0.01)。PIS患者有Hp感染者与无Hp感染者外源性β-G比较无明显差异,然而内源性β-G升高,具有统计学意义。结论PIS与Hp感染相关,并且Hp可能通过内源性β-G途径参与PIS的形成。 Objective To study pathogenic mechanism of helieobaeter pylori (Hp) infection in primary intrahepatie stones (PIS). Methods Hp DNA in bile and mueosa of PIS samples was detected by PCR while activity of β-glucuronidase ( β-G ) measured by enzymologieal method, Results PCR showed that 26 cases (70.27%) had positive Hp-DNA in bile and bile duet mucosa of 37 cases with PIS; but there was only 1 case with positive Hp-DNA in the control, with statistical significance upon infection rate of Hp between two groups (P 〈0.01 ). PIS cases included those infected with or without Hp. There was no statistical difference upon exogenous β-G activity but statistical difference upon endogenous β-G activity between positive Hp DNA and negative Hp DNA. Conclusion PIS involves Hp infection agents that may accelerate formation of PIS by enhancing activity of endogenous β-G activity,
