摘要
目的评估低强度脉冲超声波(LIPUS)治疗高能量胫骨骨折的临床疗效。方法2004~2005年, 60例采用切开复位内固定或使用外固定器的胫骨骨折患者被随机分为试验组30例和对照组30例。试验组采用LIPUS治疗:术后第一天起,每天治疗20min,持续90天,对照组术后未予以LIPUS治疗。结果术后平均随访10个月。试验组胫骨骨折愈合时间为9~15周(平均12周),无延迟愈合和骨不连发生;对照组为13~36周(平均18周),延迟愈合3例(愈合时间为8~9个月)。按Johner-Wruhs标准评估,术后12周和26周,试验组优良率分别为83%和100%,对照组为47%和87%,两组间有显著性差异(P<0.01)。结论LIPUS是一种促进骨折愈合的有效方法。
Objective To evaluate the clinical effect of high energy tibial fracture with low intensiy pulsed ultrasound (LIPUS). Methods Between 2004 and 2005, sixty patients with tibial fracture randomly assigned to the treatment group with LIPUS (n=30) or the control group (n=30), whom had been immobilized by either internal or external fixations, were followed up. Patients in the treatment group were provided with LIPUS device (Nexsound), and the treatment was given with 20 min/day for 90 days from the first day postoperation. Those of the control group were not given special treatment. Results The cases were followed up from 5 months to 14 months, with an average of 10 months. The mean time to fracture healing was 11 weeks (8-15 weeks) in the treatmental group, and was 18 weeks (13-36 weeks) in the control group. In the treatment group, no delayed union or nonunion, while 3 fractures (10%) healed spontaneously from 8 and 9 months in the control group. According to the criteria of Johner-Wruhs, by week 12 and 26 postoperation, there were rate 83% and 100% in the treatmental group, 47% and 87% in the control group respectively, significant difference was found between the two groups. Conclusion LIPUS is an effective method to treat fractures.
