血浆N-末端脑钠肽原与老年慢性心力衰竭患者心功能相关性研究 被引量:5

Correlation between plasma N-terminal pro-brain natriuretic peptide and cardiac function in elderly patients with chronic heart failure.

摘要 目的探讨老年慢性心力衰竭(CHF)患者血浆N-末端脑钠肽原(NT-proBNP)与心功能的关系及与左心室质量比值(PLM)的相关性。方法选择老年CHF患者225例,以纽约心脏病心功能分级(NY- HA)方法评估心功能,60例体检健康者作为对照,采用电化学发光双抗体夹心法测定血浆NT-proBNP水平;超声心动图测定CHF组及健康组左心室质量,并根据收缩压计算左心室质量预计值。左心室质量比值(PLM)=左心室质量实测值/左心室质量预计值×100%。结果老年慢性CHF组的血浆NT-proBNP水平及PLM显著高于健康组(P<0.05),NT-proBNP水平随着心功能NYHA分级程度加重而显著增加,其与左心室射血分数(LVEF)呈负相关(r=-0.34,P<0.01),与PLM呈正相关(r=0.79,P<0.01),CHF合并心房颤动组NT-proBNP明显高于CHF无心房颤动组(P<0.05)。结论老年CHF患者NT-proBNP水平明显升高,且随着心功能NYHA级别的增高相应递增,反映心室重构的PLM异常,NT-proBNP与PLM呈正相关,CHF合并心房颤动者NT-proBNP明显高于CHF无心房颤动者。 Objective To investigate the correlation between plasma N-termlnal pro-braln natriuretic peptide ( NT-proBNP) and cardiac function, and the percent left ventricular mass ( PLM ) in elderly patients with chronic heart failure( CHF). Methods New York Heart Association (NYHA) Functional Classification was used to evaluate 225 elderly patients with CHF. Among them,60 healthy volunteers were as controls. Plasma NT-proBNP level was detected with Electrochemistry sandwich immunoassay and the left venficular mass of both groups were measured with ultrasonic cardiogram. The predicted left ventricular mass was calculated according to the systolic blood pressure. Resuits NT-proBNP and PLM of elderly patients with CHF were higher than that of control group ( P 〈 0.05 ). The NT-proBNP level 'increased with the lever of NYHA, and was negatively correlated with LVEF ( r = - 0.34, P 〈 0. 01 ) and positively correlated with PLM ( r = 0.79, P 〈 0.01 ). NT-prcBNP level was higher in heart failure patients with atrial fibrillation than that of heart failure patients without atrial fibrillation ( P 〈 0.05 ). Conclusion Plasma NT-proBNP level is increased in elderly patients with chronic heart failure and correlated positively with the heart failure classification and PLM. NT-proBNP level in heart failure patients with atrial fibrillation is higher than that of heart failure patients without atrial fibrillation.
