双腔循环宫腔冲洗法获取早孕胎儿滋养细胞方法研究 被引量:1

摘要 目的:探索一种适合于孕早期产前诊断的、安全无损伤的获取胎儿细胞的取样方法。方法:①制备双腔循环宫腔冲洗管,对拟行人工流产的71例早孕妇行循环子宫腔冲洗,研究获取胎儿滋养细胞的情况及取样时间。②对获取的滋养细胞行免疫组化鉴定。③对拟行药物流产的早孕妇行循环子宫腔冲洗,探索取样操作对孕妇及胎儿的短期影响。结果:双腔循环宫腔冲洗法获取滋养细胞的例数迭73.9%;最佳取样时间为妊娠41~70d;APAAP免疫组化试验表明,多核细胞能与抗滋养细胞多克隆抗体及单克隆抗体发生强阳性反应;循环宫腔冲洗取样操作短期(3d)内观察未发生流产、感染、腹痛等异常情况。结论:①孕早期在宫腔内可以收集到胎儿滋养细胞。免疫组化研究进一步证实它的性质。②双腔循环宫腔冲洗法取样简便易行,效果比较满意。③最佳取样时间在孕41~70d。④循环宫腔冲洗法短期观察对母儿是安全的。⑤双腔循环宫腔冲洗法在同类研究中首次采用,对孕早期产前诊断取样方法的突破是一种有益的探索。 Objective:To investigate a non--invasive, safer sample collection method for early prenatal diagnosis of trophoblastic cells. Methods: (1)A flexible pipe with double cavity was prepared. In the termination of pregnancy by induced abortion, 71 samples were collected with this device which allowed normal saline liquid to flow in cycle through the uterine cavity to study the trophoblastic cells and the sample--collected time. (2)Immunohistochemistry was carried out to determine the feature of positive cells.(3)Samples from patients who would terminate pregnancy with medicine were retrieved with a double cavity flexible pipe, and the saftey of operation in short time was observed. Results :We obtained more liquid and trophoblastic cells with a double cavity flexible pipe than with a single cavity pipe (73. 9%, 44. 8%, P〈0.05). The best stage of sample collection was at 41 to 70 days of gestation. APAAP immunohistochemistry strongly responded to positive cells. We didn't find any harmful effect of this sample collection on pregnant woman and foetus in short time (3d),such as abortion,infection, abdominal pain, etc. Conclusion: (1) Trophoblastic cells can be found in the low part of the uterine cavity. Immunohistochemistry indicates the feature of cells. (2)A double cavity flexible pipe is more reliable than a single cavity pipe in sample collection. (3)The best stage of sample collection is at 41 to 70 days of gestation. (4)Retrieving sample from uterine cavity by a double cavity flexible pipe is safe in short time. (5) The method of collecting intrauterine lavage fluid with a double cavity flexible pipe is first time used. it is a good test in finding a method of sample collection for early prenatal diagnosis of trophoblastic cells.
