头孢吡肟对铜绿假单胞菌和鲍曼不动杆菌体外抗菌活性研究 被引量:1

摘要 目的了解头孢吡肟对铜绿假单胞菌和鲍曼不动杆菌的体外抗菌活性。方法常规培养分离细菌,应用VITEK微生物全自动分析仪和API鉴定系统鉴定细菌,药敏试验应用K-B纸片法和肉汤倍比稀释法,按NCCLS标准进行。结果125株铜绿假单胞菌和鲍曼不动杆菌主要来源于呼吸道标本(占80.0%),产AmpC酶的阳性率分别为25.3%和10.0%,铜绿假单胞菌和鲍曼不动杆菌对亚胺培南的耐药率分别为40.0%和6.0%,对头孢吡哌酮/舒巴坦耐药率分别为17.3%和26.0%,头孢吡肟对两种菌的最低抑制浓度(MIC50)为8.0 mg/L和16.0 mg/L、MIC90为32.0 mg/L和>64.0 mg/L,抑菌率分别为61.3%和48.0%。结论对两种细菌引起的重症感染应根据药敏试验结果首选头孢哌酮/舒巴坦或亚胺培南治疗。头孢吡肟对铜绿假单胞菌的体外抗菌活性近2年比较稳定,临床应加强该药物的管理和合理应用。 Objective To understand the antibacterial activities of cefepime against Pseudomonas aeruginosa and Acinetobactei baumannii in vitro. Methods The bacteria were isolated and cultured using routine procedures. All isolates were identified with the full-automatic microbial analyzer (VITEK) and API bacterial identifying system. Drug sensitivity tests were determined by Kirby-Bauer disk diffusion and broth dilution methods in accordance with NCCLS recommendations. Results Most of 125 strains isolated were predominantly of respiratory tract origin (80.0%), The zymogenic rates of Pseudomonas aeruginosa and Acinetobaeter baumannii were 25.3% and 10.0%. Resistance to Imipenem and cefoperzone/subactam was found in 40.0% and 17.3% of Pseudomonas aeruginosa strains, respectively, as was in 6.0% and 26.0% of Acinetobacter baumannii strains. With cefepime, MICs0 against Pseudomonas aeruginosa and Acinetobacter baumannii was 8.0mg/L and 16.0mg/L, and MIC90 was 32.0mg/L and 〉64.0mg/L respectively. The rates of inhibition were 61.3% and 48.0%. Conclusion The use of Imipenem or cefoperazone/subactam in severe forms of respiratory infection should be primarily considered and based on drug sensitivity test. The in vitro activity of cefepime against Pseudomonas aeruginosa remains stable over the past two years. However, rational use of this drug is justified in ciinical practices.
