摘要
目的探讨多药联合治疗小儿慢性再生障碍性贫血的效果。方法选择35例慢性再生障碍性贫血患儿,给予四种药物联合治疗:①静脉用丙种球蛋白1g/kg,每月一次,共6次;②免疫抑制剂:环胞菌素A5～8mg/kg.d,一日剂量分早晚两次口服,疗程至少6个月;③雄性激素:康力龙0.1～0.2mg/kg.d,一日剂量分三次口服,疗程至少12个月以上;④中药:补肾养血中药经辨证后加减汤剂或复方皂矾丸,疗程至少6个月以上;并随访3～12个月观察血常规、骨髓象的变化。结果3、6、12个月时有效率分别为74.3%、80%、85.7%;四种药物联合治疗小儿慢性再生障碍性贫血总有效率达到85.7%。结论多药联合治疗小儿慢性再生障碍性贫血效果比较显著,值得推广治疗慢性再障。
Objective To investigate the clinical drugs in childhood chronic aplastic anemia. Methods efficacy of the combinational therapy of multiple Intravenous immumoglobulin was injected once a month for 6 times at the dosage lg/kg; cyclosporine A was orally administrated averagely twice a day at the dosage 5 - 8 mg/kg d for at least 6 months; Androgen was orally given equally three times per day at the dosage 0. 1 -0. 2 mg/kg d for at least 12 months; Chinese herbs were chosen as kidney-nourishment and blood-nutriment agents for at least 6 months; 3 to 12 months following up was carried out to examine the change of blood routine and bone marrow aspiration. Results The effective rations were 74. 3%, 80% and 85.7% at 3^rda, 6^th and 12^th time points, respectively. The gross effective ratio of intergrated four drugs was up to 85.7% in children with chronic aplastic anemia. Conclusion For childhood chronic aplastic anemia, the combinational therapy of multidrugs is significantly effective and worthy of wider adoption.
