摘要
目的:研究微波热疗联合应用化疗药物,对126例晚期肿瘤患者的疗效观察。方法:以UHR-2000型高能聚束微波热疗机进行腹腔部区域加温并联合化疗,辐射器距皮肤25cm,输出功率为500-800W,持续加温120~160rain,全程监测血压、血氧、心电图、呼吸。肺小细胞肺癌以EP方案化疗;非小细胞肺癌为NP方案;胃癌为DF+HCPT方案;肠癌主要是FOLFOX方案;乳腺癌为CAF或NP方案;胰腺癌为GF方案;食管癌FVP方案。每次化疗周期间隔3~4周,每次热疗间隔3d。热疗加温后体温达39.5℃以上时将当天化疗药物从静脉给药。结果:126例患者总有效率20.63%,与对照组相比差异有统计学显著性(P〈O.01);疗效与生存期以一个化疗周期行4次热疗优于一个化疗周期行2次热疗者。结论:UHR-2000型高能聚束微波热疗机进行全身热疗联合化疗治疗晚期肿瘤患者安全有效。
Objective:To observe the curative effect of high power focused-beam microwave-based whole body hyperthermia combined with chemotherapy on 126 cases with advanced cancer. Methods: One hundred and twenty six patients with advanced cancer received abdominal hyperthermia using UHR-2000 high-energy-beam microwave thermotherapeutic device combined with chemotherapy. The temperature of treatment was 40-42℃. The pulse generator was placed 25 cm away from skin. The heating power was set at 500-800 W and the overall heating lasted for 120-160 rain. The blood pressure, blood oxygenation, electrocardiogram, and breathing changes were monitored during the whole period. The EP regimen (cisplatin and etoposid) was used for small cell lung cancer; NP regimen (cisplatin plus vinorelbine) for non small cell lung cancer; DF+ HCPT regimen for gastric carcinoma; FOLFOX regimen (5-fluorouracil and folinic acid) for colorectal cancer; CAF (cytoxan+ adriamycin+ 5-fluorouracil) or NP regimen for breast carcinoma; GF regimen for pancreatic cancer; and FVP regimen (cisplatin+vinblastine+ fluorouracil) for esophageal carcinoma. The interval was 3-4 weeks for each cycle of chemotherapy and 3 d for thermotherapy. If the body temperature was higher than 39.5 ℃ after thermotherapy, the chemotherapeutic agents were administered intravenously. Resuits:The overall response rate of 126 patients was 20.63% (complete remission plus partial remission). The difference was significant compared with control group who only received chemotherapy (P〈0.01). Four times treatment in one cycle resulted in higher efficacy and longer survival time compared with only twice treatment in one cycle. Conclusion: High power focused-beam microwave (UHR-2000)-based whole body hyperthermia combined with chemotherapy is effective for the treatment of advanced tumor.
