颈椎后纵韧带骨化非手术治疗脊髓功能状态评估(63例回归分析) 被引量:2

Spinal cord function in patients with ossification of cervical posterior longitudinal ligament after at least two years nonoperative treatment:a regression of the outcomes in 63 cases

摘要 [目的]探讨症状较轻的颈椎后纵韧带骨化患者的非手术治疗效果。[方法]河北北方学院第一附属医院自1995年1月以来非手术治疗108例颈椎后纵韧带骨化患者,其中63例症状较轻患者(JOA评分>12分)获2年以上的随访,作者以非手术治疗前、后脊髓功能状态的JOA评分为观察标准,将疗效与多种可能的相关因素进行多重线性回归分析。[结果]治疗前平均JOA评分为(12.22±1.52)分,随访平均JOA评分为(14.85±2.20)分,治疗前、后的JOA评分平均变化率为(58.09±11.9)%。经回归分析,治疗前JOA评分与疗效呈正相关,治疗前、后脊髓功能状态存在线性回归关系。[结论]脊髓功能损害较轻的患者(JOA评分>12分)经2年以上非手术治疗疗效较好。 [Objective] To investigate the effects of nonoperative treatment on the ossification of cervical posterior longitudinal ligament (OPLL) patient who had lighter symptom. [Method] There have been 108 ossification of cervical posterior longitudinal ligament treated in the First Affiliaed Hospital of Hebei Northern University since January 1995, 63 patients treated by nonoperative treatment (JOA scores 〉 12). They were followed up above 2 years, by means of Japanese Orthopaedic Associaion (JOA) scores before and after nonoperative treatment, many relevant factors and the outcomes were analyzed by maltinomial linear regression. [Result] The average JOA scores was (2. 22 ± 1.52) points before treatment and ( 14. 85 ± 2. 20) points after treatment. The average recovery rate was 58. 09% ± 11.9%. According to the results of regression, the JOA scores were positively correlated with the effect of the nonoperative treatment, cervical spinal function before and after treatment exist multinomial linear regression. [ Conclusion] The treatment of OPLL patients whose cervical spinal function was lighter (JOA scores 〉 12) is good after nonoperative treatment above 2 years.
