摘要
目的观察肺心病急性加重期患者血小板活化功能的变化,以评估注射用丹参对肺心病患者血小板活化状态及临床疗效。方法65例慢性肺心病急性加重期患者随机分为两组：A组33例,进行常规治疗；B组32例,在常规治疗基础上给予注射用丹参800mg/d静脉滴注,疗程均为15d,观察患者的症状和体征变化,并于治疗前后应用流式细胞仪测定外周血小板CD62P及CD63,行心脏超声检查测定平均肺动脉压(mPAP),比较两组治疗前后症状、体征和检测的指标。结果治疗后,B组患者血小板活化指标CD62P、CD63及mPAP明显降低(P＜0．01),A组患者上述指标无明显改善(P＞0．05),直线相关分析表明CD62P、CD63与mPAP的水平呈正相关。两组患者的临床疗效有显著性差异(P＜0．05)。结论肺心病患者血小板活化增强在肺心病发病中起一定作用,注射用丹参可有效抑制肺心病患者血小板活化功能,从而降低肺动脉高压,提高临床疗效。
Objective To observe the serial levels of the CD62p and CD63, so as to evaluate the effect injecting Radix Salviae Miltiorrhizae on the platelet activity and clinical treatment of patients with pulmonary heart disease.Methods The 65 patients with pulmonary heart disease were randomly divided into group A（33 patients ）and group B（32 patients）. The patients of group A were treated by the routine therapy,The patients of group B were treated by the combination of routine therapy and the injecting Radix Salviae Miltiorrhizae （800mg,iv per day）for 15 days.The symptoms and signs were observed,the patients,serial levels of the CD62p and CD63 were examined by flow cytometric immunology respectively,mPAP was detected by ultrasound cardiography before and after the treatment, The results of the pre- and the post-treatment were contrasted.Results Before treatment,the serial levels of the CD62p and CD63 of the patients of group A were higher significantly than those in controlled group.After treatment,in group B,the blood CD62P, CD63 and mPAP were significantly lower （P〈0.01）;in group A,these index slightly improved（P〉0.05）.Furtbermore,the levels of CD62P and CD63 were positively related with that of mPAP;In group B and group A,the significant difference was found between the two groups in clinical effects （P〈0.05）.Concluslon The platelet activity may be involved in pathogenesis of cor pulmonale;the injecting Radix Salviae Miltiorrhizae can inhibit the patients＇ platelet activity,lower the hypertension of pulmonary arteries and improve clinical therapeutic effect.
