摘要
目的:比较椎体间植骨对腰椎内固定棒应力分布的影响,为减少腰椎内固定棒在临床应用中的疲劳提供力学依据。方法:选用6具人新鲜腰椎标本,置入脊柱通用椎弓根螺钉系统(AO/ASIF的USS系统)依次测定保留椎间盘、去除部分椎间盘以及椎间植骨后固定棒的应力,并对三种情况下腰椎内固定的应力分布进行分析比较。结果:去除部分椎间盘组USS系统内固定棒承受应力最大,保留正常椎间盘组最小,而椎间植骨组介于二者之间,应力小于去除部分椎间盘组(P<0.01),大于保留正常椎间盘组(P<0.01)。结论:椎间植骨对内固定棒具有有效的抗疲劳保护作用,为腰椎固定提供有效保证,支持临床上减压后椎间植骨内固定对腰椎失稳治疗的应用。
Objective:To compare the impact of bone graft in intervertebral disc on load distribution after instrumentation,and to provide the mechanical gist into the fatigue status induced by instrumention.Method: Six fresh samples of human lumbar vertebras were placed with the universal spine system (USS).Stress along the USS for intact intervertebral disc(group A),partial removal of intervertebral disc(group B) and bone graft in intervertebral disc (group C) were measured respectively.Result:The USS in group B shared more stress than in group C ;the USS in group C shared least stress during all phase of movement.Conclusion: Bone graft in intervertebral disc can provide effective protection to the instrument itself which can be used to treat lumbar distabilization due to decompressional surgery.
