摘要
目的 分析杭州地区孕中期妇女运用甲胎蛋白。(AFP)和游离绒毛膜促性腺激素β亚基(Free-βhCG)进行胎儿先天缺陷产前筛查的结果;并据此修正两筛查指标的孕周中位数值。方法对杭州地区孕14—20W的妇女进行产前AFP和Free-βhCG的检测。胎儿先天畸形由染色体核型分析、B超检查确诊,或经引产证实。应用非线性加权回归求得本地区两筛查指标的中位数,并计算中位倍数值进行分布拟合。比较标化前后检测效果。结果8516例受检孕妇,共筛查出唐氏综合征3例、爱德华综合征2例,神经管畸形7例,其他胎儿异常17例;杭州地区孕中期妇女AFP及Free-β-hcG中位数比欧洲妇女分别高18%和14%;用修正后的中位数重新分析,孕中期假阳性率降低。结论血清AFP、Free-βhcG联合检测,可作为杭州地区孕中期妇女产前筛查优选项目。而根据地域人群的差异,对MuhiCalc软件内嵌中位数进行修正后更适于杭州地区孕中期妇女的产前筛查。
Objective: To analysis the antenatal screening for fetal congenital abnormality in 8516 mid-pregnant women in Hangzhou with alpha fetoprotein (AFP) and free chofionic gonadotropin beta subunit (Free-βhCG), and recorrect the median of the both marks. Methods: Serum AFP and Free-βhCG of all cases with 14 -20 weeks pregnancy were detected. Fetal congenital abnormality was diagnosed with chromosomes examination and B-type ultrasonography or inducing abortion. The median of both the marks were recalculated by nonlinear weight regression, and according to the median, the Multiple of Median (MOM) was also accounted and fitted. The detecting effects of MoM were compared before and after correction. Resuhs: From 8516 cases, 3 Down's syndrome, 2 Edward's syndrome, 7 neural tube defects and 17 other fetal abnormalities were acreened. AFP and Free-βhCG-MOM of the cases in the pregnant metaphase in Hangzhou increased 18% and 14% than those of pregnant women in European. False positive ratio of antenatal screening results by the recorrected MoM markedly decreased. Conclusions: The combining detection of serum AFP and Free-βhCG can be considered as the first examination for the antenatal screen of women in the pregnant metaphase in Hangzhou. Instead of MoM in MultiCalc software, the corrected MoM according to the character of the crowd in Hangzhou is more suitable for the antenatal screen.
